Tenders Are Invited For Complex Care Fps For Individuals Resident In, Or Funded By St Helens

The Support Is Based On A Recovery And Rehabilitation Focused Model, With All Service Users Receiving A Core Package Of Support From A Staff Group. Support Received Will Be Delivered As Stipulated In Each Service Users Social Care Assessment And Individual Care Plan. Each Service User Will Also Require An Outcome Planner To Evidence Their Support/ Rehabilitation Package Which Enables Them To Gain The Skills Required To Live Independently In The Community. It Is Expected That The Successful Provider Will Work Closely With Adult Social Care And Commissioning To Support Reintegration Into The Wider Community.The Service Will Provide Support In Addition To The Existing Support Delivered Under The Community Mental Health Framework (Cmhf). It Does Not Replace Existing Commissioned Services.
