The Subject Of The Order Is The Supply Of Medicines And Their Delivery To The Pharmacy Department In The Assortment And Quantities Specified In Annex No. 2 To The Idw - Assortment Form - Quantitative. 2. The Offered Subject Of The Order Must Meet The Requirements Of The Ordering Party Specified In The Assortment And Quantity Form, Be Brand New And Ready For Use As Intended Without Making Additional Purchases.3. It Is Possible To Replace The Oral Form - E.G. Tablet. Capsule, Coated Tablet, Dragee And Vice Versa.4. It Is Allowed To Replace The Injection Form, E.G. Vial, Amplifier, Bottle, Bottle, Etc. And Vice Versa.5. Shortages/End Of Production Should Be Valued At The Last Sale Price And A Note Added Under The Package. 6. The Offered Subject Of The Order Must Be Authorized For Marketing And Use As A Medicinal Product. Medicinal Products Constituting The Subject Of The Order Should Be Understood As Medicinal Products Within The Meaning Of The Pharmaceutical Law Act Of September 6, 2001 (Consolidated Text: Journal Of Laws Of 2024, Item 686). The Offered Medicinal Products Must Be Admitted To Trading In Poland In Accordance With The Principles Set Out In Art. 3 Or 4A Of The Pharmaceutical Law.
