The Subject Of The Order Is An Annual Service Inspection And Maintenance Of The Gamma Hyperbaric Chamber Systems, Hereinafter Referred To As The Chamber, Including Removal On Site Or, If Impossible, At The Contractors Premises, During The Inspection Of All Defects In The So-Called Chamber Systems, And Preparation And Delivery Of The Acceptance Protocol For The Above-Mentioned. Services Along With All Necessary Documents Referred To In §5 Section 3 Of The Agreement Template.2. Cpv: 50421000-2 Repair And Maintenance Services Of Medical Equipment3. A Detailed Description Of The Subject Of The Order Is Attached As Annex 1 To The Contract Template Entitled List Of Service Activities.4. The Contractor Is Obliged, In Particular, To: - Have The Necessary Tools And Service Codes To Carry Out The Complete Scope Of Service Activities, Including Updating The System, If Required By The Manufacturer, - Preparing Documentation Of The Inspection Activities Performed; - Completing The Technical Passport, In Accordance With The Act Of April 7, 2022 On Medical Devices. Equipment In Good Working Order And Safe For Use, Date Of Inspection, List Of Activities Performed And Parts Replaced, Date Of Next Inspection, Contractors Stamp And Signature. In The Absence Of A Passport, Issues A New Passport And Fills It Out In Accordance With Its Content. Passport - Job Form Pu/A-100; - Issuing A Written Certificate Of Operational Capability For The Hyperbaric Chamber System, - Issuing A Written Guarantee Of Efficiency Granted For The Hyperbaric Chamber System. 5. In The Event Of A Negative Result Of The Inspection, The Contractor Is Obliged To Make An Entry In The Technical Passport Of The Equipment, Indicating That The Device Is Not Operational, Specifying The Cause Of The Malfunction, And Placing A Clear Information On The Defective Equipment: Device Defective - Do Not Use, Or Device Intended For Repair - Do Not Use. In Such A Case, The Contractor Is Obliged To Issue An Opinion/Expert Opinion Regarding The Possibility Of Removing The Cause Of The Malfunction And Any Related Expenses That May Be Incurred By The Ordering Party.6.
