The Subject Of The Order Is The Provision Of Services In The Field Of Transport Of Dialysis Patients In A Sitting Position From Their Place Of Residence To The Dialysis Center Located At The Premises Of The Ordering Party In Szczecin At Al. Powstańców Wlkp. 72 (Exit From Szpitalna Street) And From The Dialysis Center To The Patients Place Of Residence. 2. A Detailed Description Of The Requirements Regarding The Subject Of The Order And The Conditions Of Its Implementation Is Included In Chapter Ii Of The Swz. 3. The Order Consists Of 1 Task. 4. The Ordering Party Does Not Allow The Submission Of Variant Offers. 5. Determining The Requirements Regarding The Employment Of Persons By The Contractor Or Subcontractor On The Basis Of An Employment Relationship: A) Requirement Resulting From Art. 29(3A) Of The Public Procurement Law: The Ordering Party Requires That The Contractors Staff Carrying Out The Order Directly In The Field Of Patient Transport - I.E. Drivers And Drivers Assistants (If Applicable) - Be Employed By The Contractor Or Subcontractor Under An Employment Contract. A Detailed Description Of The Subject Of The Contract In This Respect Is Provided In Chapter Ii Of The Swz. B) During The Execution Of The Contract, The Ordering Party Has The Right To Control The Contractors Fulfillment Of The Condition Of Employment Under The Employment Contract Of The Persons Referred To In Point (A). And); C) The Method Of Documenting Employment Under The Employment Contract By The Contractor And The Type Of Sanctions Towards The Contractor For Failure To Meet This Condition Are Specified In Detail In The Contract, The Proposed Provisions Of Which Are Included In Chapter Iii Of Swz 6. The Ordering Party Requires That The Contractor Has A Civil Liability Insurance Policy In The Field Of Business Activity Which Is The Subject Of This Order, Covering Contractual And Tort Liability For Property And Personal Damage And Their Consequences For The Total Amount Warranty Not Lower Than Pln 100,000.00. 6. The Ordering Party Requires The Implementation Of The Subject Of The Order For A Period Of 24 Months From The Date Of Commencement Of The Provision Of Services. Approximate Implementation Period: From September 1, 2026 To August 31, 2028.
