1. The Subject Of This Order Are Construction Works Related To The Implementation Of The Task Titled: Reconstruction Of District Road No. 4586P On The Florentyna-Goliszew Section.2. The Order Is Being Carried Out As Part Of The Task Entitled: Reconstruction Of District Road No. 4586P On The Florentyna-Goliszew Section, Co-Financed By The Government Road Development Fund.3. The Detailed Scope Of The Order Is Specified In Annex No. 1 To The Swz (Design Documentation) And Annex No. 2 To The Swz (Offer Cost Estimate). The Subject Of The Order Includes The Reconstruction Of District Road No. 4586P On The Florentyna-Goliszew Section, With A Length Of 7,457 Km. As Part Of The Implementation Of The Subject Order, The Following Will Be Performed: 1) Preparatory Works And Construction Services, 2) Demolition Works, 3) Earthworks, 4) Road, 5) Pedestrian Roads And Exits, 6) Shoulders, 7) Drainage, 8) Roundabout Type Intersection, 9) Vertical Markings And Horizontal,10) Technological Channel,11) Post-Construction Geodetic Inventory.4. The Ordering Partys Requirements Regarding The Employment Of Persons Under An Employment Contract By The Contractor Or Subcontractor: 1) The Ordering Party, Pursuant To Art. 95 Section 1 Of The Public Procurement Law Requires The Contractor Or Subcontractor To Employ, Under An Employment Contract, Persons Performing Activities Related To The Execution Of The Order, The Performance Of Which Consists In Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974 Labor Code (Journal Of Laws Of 2025, Item 277, As Amended). This Requirement Applies To Manual Workers, Drivers Of Means Of Transport, Operators Of Machinery And Equipment;2) The Contractor Is Obliged To Include In Each Subcontracting Agreement Appropriate Provisions Obliging Subcontractors To Employ Under An Employment Contract All Persons Performing The Above-Mentioned Activities;3) Within 14 Days From The Conclusion Of The Contract, The Contractor Is Obliged To Submit To The Ordering Party A Declaration That The Persons Performing The Activities Specified In Point 1 Are Employed Under An Employment Contract Within The Meaning Of The Provisions Of The Act Of June 26 1974 Labor Code, Including Proof Of, Among Others: Names And Surnames Of Employees, The Number Of These People, Type Of Employment Contract, Full-Time Employment And Position (Scope Of Activities Performed); 4) During The Execution Of The Order, At Each Request Of The Ordering Party Within The Deadline Specified In The Request, The Contractor Will Submit To The Ordering Party Copies Of Anonymized Employment Contracts Certified As Originals By The Contractor Or Subcontractor, Respectively, In A Manner Ensuring The Protection Of Individual Employees Data, In Accordance With The Provisions Of The Gdpr [Regulation Of The European Parliament And Of The Council (Eu) 2016/679 Of April 27, 2016 On The Protection Of Natural Persons With Regard To The Processing Of Personal Data And On The Free M
