Tenders Are Invited For Delivery, Installation And Commissioning Of An Ultrasound Device For Szpzlo Warszawa Praga-Północ

1. The Subject Of The Order Is The Purchase, Delivery And Installation Of A Brand New (Manufactured In 2026), Unused Ultrasound Device With Accessories And Warranty Service For The Internal Medicine And Specialist Clinic At Ul. Jagiellońska 34 In Warsaw. 2. The Delivered Device Must Be Brand New, Manufactured After January 1, 2026, Unused, Fully Operational And Complete, Meeting All The Requirements Of The Ordering Party Specified In The Description Of The Subject Of The Order In Terms Of The Required Technical And Functional Parameters, Which Are Specified In Annex No. 2 To Swz.3. The Contractors Obligations Will Include At Least: 1) Ensuring, At Its Own Expense And Risk, The Transport Of The Apparatus/Equipment (Subject Of The Order) As Well As Unloading And Internal Transport To The Place Indicated By The Ordering Party, 2) Connecting, Configuring And Commissioning The Apparatus/Equipment Constituting The Subject Of The Order, 3) Delivering The Product (Equipment) Complete, Brand New, Not Ex-Exhibition, 4) Collecting And Disposing Of Packaging After Delivery At Its Own Expense Equipment/Apparatus.4. Delivery, Assembly And Commissioning Of The Apparatus/Equipment May Take Place Between 8:00 A.M. And 3:00 P.M., From Monday To Friday, Excluding Public Holidays.5. Detailed Conditions For The Implementation Of The Order Are Specified In The Draft Provisions Of The Contract Constituting Annex No. 6 To Swz.6. The Contractor Will Include In The Price For The Execution Of The Subject Of The Order All Costs Related To The Execution Of The Order, Including Costs Resulting From The Requirements Specified In The Swz, Annex No. 2 To The Swz And Annex No. 6 To The Swz.7. The Ordering Party Does Not Allow The Offer Of A Reconditioned Or Demonstration Device. 8. The Ordering Party Requires The Delivery Of Complete Equipment With Documentation In Polish.9. All Requirements Specified In The Documents Indicated Above Constitute The Minimum Requirements Of The Subject Of Delivery, And Their Fulfillment Is Obligatory. Failure To Meet The Above-Mentioned Minimum Requirements Will Result In Rejection Of The Offer As Inconsistent With The Terms Of The Contract Pursuant To Art. 226 Section 1 Point 5 Of The Public Procurement Law. 10. The Ordering Party Requires Staff To Be Trained In The Operation Of The Supplied Equipment From 8:00 A.M. To 12:00 P.M., Monday To Friday - The Training Date Must Be Agreed No Later Than 6 Days Before The Start Of The Training.11. The Ordering Party Allows In Accordance With Art. 462 Of The Public Procurement Law, Entrusting The Order To The Contractors Subcontractors.12. The Ordering Party Requests That The Contractor Indicate In The Offer The Parts Of The Order The Performance Of Which He Intends To Entrust To Subcontractors, And Provide The Names Of Any Subcontractors, If They Are Already Known In Accordance With Art. 462 Section 2 Of The Act Pzp.13.
