1. The Subject Of The Order Is The Provision Of Individual Rehabilitation Services At The Place Of Residence Of The Participants Of The Local Support Force Project.2. Providing Individual Rehabilitation Services At The Place Of Residence Of The Participants Of The Local Support Force Project In The Amount Of 2,400 Clock Hours: 1) Monthly Number Of Hours - 120 Hours/Month For 10 Participants Indicated By The Ordering Party, 2) Period Of Services Provided - 20 Months. 3. People Covered By Home Rehabilitation Are People Who Need Support In Everyday Functioning (Including Due To Age, Health Condition, Disability) And Their Environment, Including People With Significant Or Moderate Degrees Of Disability.4. The Scope Of Services Referred To In Section 1, Includes Conducting A Preliminary Diagnosis With Each Project Participant, Determining Rehabilitation Treatments Based On Individual Needs And Expectations And Performing Them In The Place Of Residence.5. Classes With A Rehabilitator Are Aimed At General Assistance In The Fitness Of Participants, So That They Can Stay In The Local Community As Long As Possible, To Improve And Maintain Their Physical Fitness And Mental Condition At The Current Level. The Scope Of Support Does Not Coincide With That Financed By The National Health Fund. The Scope Of Rehabilitation Will Not Be Curative, But Preventive. Its Main Goal Will Be To Improve The Quality Of Life. Preventive Rehabilitation Is Not Focused On Treating The Effects Of Diseases, But Primarily On Preventive Measures And Early Intervention.6. The Contractors Responsibilities Will Include: 1) Diagnosing Problems, 2) Conducting Rehabilitation Activities, 3) Keeping Documentation, Including: Work Sheets With The Project Participant, 4) Performing The Activities That Are The Subject Of The Contract With Due Diligence, 5) Supervising The Correct Implementation Of This Order, 6) Providing Services In Accordance With The Schedule, 7) Submitting, By The Last Day Of Each Month, A Summary Of The Services Provided, Taking Into Account The Number Of Hours Allocated For Rehabilitation And The Number Of People Who Took Advantage Of It.7. The Ordering Party Emphasizes That The Number Of Hours Of Service Provision Should Be Understood Only As The Actual Time Of Service Provision, Excluding Preparatory Activities, E.G. Travel To Project Participants.8. Rehabilitation Services May Be Provided Five Days A Week, Monday To Friday, From 7:00 A.M. To 8:00 P.M. On Weekdays.9. The Contractor Must Provide His Own Equipment Necessary To Properly Provide The Service, Such As: Exercise Equipment, Medical Pads, Gloves, Oils, Creams And Other Necessary Equipment. The Contractor Is Obliged To Provide Ecological Personal Protective Equipment On His Own.10. The Cost Of Travel To The Project Participants Place Of Residence Is Borne By The Contractor.11.
