The Subject Of The Order Is The Provision Of Psychological Services For Participants Of The Together For Activity Project, As One Order Ensuring Continuity And Continuation Of Work And Psychological Interactions With The Person. The Subject Of The Order Is To Conduct Individual Consultations (Action 1) By A Psychologist For 19 People - Project Participants - Adults With Disabilities (People With Intellectual Disabilities And People With Mental Illness) For 2 Hours For Each Participant, A Total Of 38 Hours (1 Hour = 45 Minutes). Individual Meetings Should Be The First Form Of Support And The Second Main Form Should Be Group Workshops (Action 2). Group Workshops For Project Participants Will Be Held For 5 Groups: 1St Group - 3 People 2Nd Group - 4 People 3Rd Group - 3 People 4Th Group - 4 People 5Th Group - 5 People The Contractor Will Conduct 10 Workshop Meetings For Each Group. One Meeting Will Last 3 Hours (1 Hour = 45 Minutes). The Participants Of Individual And Then Group Meetings Are The Same Adults - In Group I There Are People With Mental Disorders, While In Groups Ii, Iii, Iv And V There Are People With Intellectual Disabilities. It Is Important That The Contractors Activities Are Divided And Coordinated In Such A Way That The Psychologist Who Participated In Individual Consultations With A Given Person Also Conducts Group Classes With The Same People With Whom He Had Individual Consultations. Individual Consultations Will Aim To Determine The Strengths And Weaknesses Of The Participant. Project, His Skills And Difficulties In Functioning In A Social Group. The Meetings Will Be Held In: A) Wadowice - For 3 People, B) Zebrzydowice - For 4 People, C) Spytkowice - For 7 People, D) Andrychowa - For 5 People, In Places Indicated By The Ordering Party With A Frequency Of One Or Two Times A Week, From Monday To Friday. The Ordering Party Will Provide A Place That Meets The Accessibility Conditions. The Days And Times Of The Meetings Should Be Adapted To The Capabilities Of The Participants. The Contractor Submits An Offer For The Entire Subject Of The Contract (Measure 1 And Action 2). These Tasks Are Interconnected And Are Described In Detail In The Description Of The Subject Of The Contract, Constituting Annex No. 6 To The Swz.
