Tenders Are Invited For Provision Of Social Services For 2026-2027 In Accordance With The Your Support Project, Under Measure 8.5 Social Services

Part I – Care Services For 2026-2027:1. The Estimated Number Of Hours For The Entire Duration Of The Order Is 12,000 With The Option To Increase Them By A Maximum Of 4,439 Hours, And The Services Are Planned To Cover 15 People Who Need Support In Everyday Functioning.2. Services Will Be Provided 7 Days A Week (Including Sundays And Holidays) From 6:00 A.M. To 10:00 P.M. 3. The Scope Of Care Services Depends On The Situation Of The Project Participant, His Or Her Individual Needs And Will Include: A) Assistance In Meeting Everyday Life Needs; B) Hygienic Care; C) Care Recommended By A Doctor, Which Includes Care Activities Resulting From The Submitted Medical Certificate Or Medical Documentation, Supplementary To Community Nursing Care; D) Ensuring Contacts With The Environment.4. Care Services May Be Provided By Adults Who Are Not Participants In The Project, The Actual Guardian Of The Project Participant And Who Are Not Members Of The Participants Family, Legal Guardians Of The Participant And Persons Actually Living With The Project Participant Who: A) Have Qualifications To Perform One Of The Following Professions: Community Caregiver, Assistant To A Disabled Person, Nurse, Caregiver Of An Elderly Person, Medical Caregiver, Qualified Caregiver In A Social Welfare Home Or Have At Least 6 Months Of Experience In Providing Care Services, Including Professional, Voluntary Or Personal Resulting From The Role Of An Actual Guardian And Have Completed A Minimum Of 80-Hour Training In The Scope Of The Service Provided, Including First Aid Or Pre-Medical Assistance, And B) Provide Certificates Of No Criminal Record And C) Provide Information About Not Being Included In The Register Of Perpetrators Of Sexual Offenses Issued By The National Criminal Register (In The Case Of Providing Services To Minors), And Will Be Accepted By The Parent Or Legal Guardian Of The Child With Disability In Accordance With Art. 21 Of The Act Of 13 May 2016 On Counteracting Threats Of Sexual Crime And Protection Of Minors (Journal Of Laws Of 2026, Item 110) And D) Are Fluent In Polish. The Ordering Party, Through The Guardians Fluent Use Of The Polish Language, Understands Efficient And Effective Communication With People Using The Services, With People Around Them, Medical Staff And The Ordering Partys Staff, And That) They Are Physically And Intellectually Fit And Their Health Condition Does Not Limit The Ability To Provide The Services In Question (In The Case Of Having A Degree Of Disability, The Diagnosed Disability Cannot Limit The Ability To Work).5. The Contractor Is Responsible For The Appropriate Selection Of Persons Providing Services In Relation To The Health Condition And Other Circumstances Of The Persons Covered By The Services.6. An Hour Of Service Provision Means 60 Minutes, Where The Time Of Service Provision Means The Time Of Performing The Ordered Scope Of Services For 1 Project Participant (It Is Not Allowed To Provide Services To More Than 1 Project Part
