The Subject Of The Order Is The Provision Of A Service Consisting In The Collection, Transport And Management Of Waste From Street Bins, Bus Stops, Playgrounds And Dog Excrement Containers Located In The Niepołomice Commune, Together With Replacing Bags, Organizing And Maintaining Cleanliness Around The Bins And Transferring Waste In Accordance With The Hierarchy. A Detailed Description Of The Subject Of The Order And Its Implementation Is Included In The Annex To The Swz Description Of The Subject Matter Of The Order. 1. Because The Implementation Of This Order Must Be Carried Out Through Performing Activities Under The Conditions Specified In Art. 22 § 1 Of The Act Of June 26, 1974, Labor Code - For The Benefit Of The Contractor Or Subcontractor, At The Place And Time Indicated By The Contractor Or Subcontractor - The Ordering Party, In Accordance With Art. 95 Section 1 Of The Public Procurement Law Requires The Contractor Or Subcontractor To Employ Persons Performing Activities Falling Within The So-Called Direct Costs Based On The Employment Relationship. This Declaration Should Include In Particular: Precise Identification Of The Entity Submitting The Declaration, Date Of Submission Of The Declaration, Indication That The Activities Listed In Point 1 Is Performed By Persons Employed Under An Employment Contract, Indicating The Number Of These Persons, The Type Of Employment Contract And The Working Hours, As Well As The Signature Of The Person Authorized To Submit A Declaration On Behalf Of The Contractor Or Subcontractor.3.
