1. The Subject Of The Order Is The Supply Of An X-Ray Machine For The Outpatient Clinic Of Nowodworski Medical Center In Connection With The Implementation Of The Grant Project Accessibility Plus For Aos (Project Number: Fers.03.07-Ip.07-0001/23), Implemented Under The European Funds For Social Development 2021-2027 Program, Co-Financed By The European Social Fund Plus, Based On The Decision On Co-Financing The Grant Project No.: Fers.03.07-Ip.07-0001/23-00 Adopted By The Minister Of Health On December 8, 20232. Detailed Requirements Regarding The Subject Of The Order Are Specified In Annex No. 2 To The Swz - Table Of Technical Parameters.3. Delivery Of The Subject Of The Order Will Take Place At The Expense And Risk Of The Contractor, To The Point Indicated By The Ordering Party, I.E. Specialist Clinic, Ul. Paderewskiego 7, 05-100 Nowy Dwór Mazowiecki And Also Include: ◦ Installation And Assembly Of Equipment At The Ordering Partys Premises; ◦ Providing The Ordering Party With An Operating Manual In Polish; ◦ Providing A Technical Passport Containing Device Identification Data; ◦ Providing A Warranty Card For The Device; ◦ Staff Training.4. After Completing The Above-Mentioned Activities, A Delivery And Acceptance Protocol Will Be Signed, Based On Which The Contractor Will Have The Right To Issue A Vat Invoice For The Performance Of The Subject Of The Contract. The Handover And Acceptance Protocol Becomes Valid When It Is Signed By Both Parties, Without Any Reservations. 5. The Ordering Party Requires At Least 24-Month Warranty On The Entire Subject Of The Order. The Contractor May Offer A Longer Warranty Period Under Item 150 Of Appendix No. 2 To Swz - Table Of Technical Parameters - Which Is A Scored Parameter. The Warranty Period Will Start Counting When The Handover And Acceptance Protocol Is Signed Without Any Reservations By Both Parties.6. Detailed Conditions For Servicing The Equipment During The Warranty Period Are Described In § 5 Of Annex 4 To The Swz - Draft Agreement.7. The Offered Subject Of The Order Must Be Brand New, Marked In Polish And Have Instructions For Use In Polish.8. The Offered Subject Of The Order - A Medical Device Must Be Authorized For Trading And Use In Poland In Accordance With Applicable Law And Have Documents Confirming The Appropriate Conformity Assessment Procedure In Accordance With The Requirements Specified In The Act Of April 7, 2022. On Medical Devices (Journal Of Laws 2024, Item 1620) And Be Marked With The Ce Mark (Declaration Of Conformity).9. The Calculation Of The Payment Deadline For The Execution Of The Subject Of The Order Will Begin At The Moment Of Issuing Correctly Prepared Vat Invoices, Issued On The Basis Of A Delivery And Acceptance Protocol Signed Without Any Reservations By Both Parties.10. Due To The Specific Nature Of Spending The Grant Funds Indicated In Section 1, The Ordering Party Requires Settlement For The Subject Of The Order On The Basis Of 2 Invoices, I.E.: ◦ Invoice No. 1 - 20
