Special-Purpose Road Passenger-Transport Services - Providing Services In The Field Of Transport Of Disabled Children Residing In The Praga-Północ District Of The Capital City Of Warsaw To Kindergartens, Schools And Educational Institutions In The School Year 2026/2027 And 2027/2028.. 1. The Subject Of The Order Is The Provision Of Transport Services For Disabled Children Living In The Praga-Północ District Of The Capital City Of Warsaw. Warsaw To A Kindergarten, A Kindergarten Section In A Primary School, Another Form Of Pre-School Education, As Well As To A Primary Or Secondary School And Other Institutions Listed In Art. 2 Points 7 Of The Act Of December 14, 2016, Education Law (Journal Of Laws Of 2025, Item 1043, As Amended) On The Home-School-Home Route And In The Scope Of Providing Care For Children During Transport In The Period From September 1, 2026 To August 31, 2028, Subject To Section 5 And 6 Of The Draft Provisions Of The Contract Constituting Annex No. 11 To The Swz. 2. Transportation Will Cover Disabled Children Who Have A Current Certificate Of Need For Special Education, Are Included In Pre-School Education, And Fulfill The Obligation Of One-Year Pre-School Preparation, Compulsory Schooling Or Compulsory Education. 3. Delivery Will Take Place To Kindergartens, Schools And Educational Institutions Operating In The Capital City Area. Warsaw, And In Justified Cases Referred To In Art. 127 Section 15 Of The Act Of December 14, 2016 Education Law (Journal Of Laws Of 2025, Item 1043, As Amended) Also To The Nearest Special School Or Center Located In The Poviats Neighboring The Capital City. Warsaw. 4. A List Of Disabled Children Covered By Transport, Including The Name Of The Kindergarten, School Or Educational Institution And The Street Where The Kindergarten, School Or Educational Institution Is Located, As Well As The Name And Contact Details Of The Child And Parent/Guardian, Will Be Provided To The Contractor By The Ordering Party After Concluding The Agreement On A Date Agreed By The Parties Separately For The 2026/2027 School Year And The 2027/2028 School Year. Up To 95 Children Can Be Transported. 5. The Contractor Will Perform The Service In Roadworthy Motor Vehicles Adapted To Transport Disabled People, With A Minimum Of 12 Cars At A Time. 6. A Detailed Description Of The Subject Of The Order And The Requirements Regarding The Method Of Its Implementation Are Specified In: 1) Designed Provisions Of The Contract - Annex No. 11 To The Swz. 2) Description Of The Subject Of The Order - Annex No. 12 To The Swz 3) Price Form Constituting Annex No. 1 To The Swz
