Wróbla 5 In Łódź As Part Of The Funding Obtained From The Voivodeship Fund For Environmental Protection And Water Management As Part Of The Program Rationalization Of Energy Consumption In Public Buildings And Municipal Resources - 2025 Edition. The Subject Of The Order Includes Two Above. Tasks: 1) Task I) General Scope Of Work To Be Performed As Part Of The Installation Of The Pv Installation:  Stage 1 - Including The Development Of Documentation  Stage 2 - Including The Execution Of Construction Works Based On The Prepared Documentation Listed In Stage 1 2) Task Ii) General Scope Of Work To Be Performed As Part Of The Thermal Modernization Of The Building 4.2 A Detailed Description Of The Subject Of The Order Can Be Found In Annex No. 1 To The Swz And In The Contract Template In Annex No. 6A And Annex 6B To The Swz 15 Method Of Submitting Offers The Offer Consists Of The Following Documents: 15.1 Offer Form - Prepared According To The Template - Annex No. 2 To The Swz (Signed With A Qualified Electronic Signature). 15.2 Price Form - Prepared According To The Template - Annex No. 2A To The Swz (Signed With A Qualified Electronic Signature) 15.3 Contractors Declaration Regarding The Grounds For Exclusion Under Art. 5K Of Regulation 833/2014 Regarding Restrictive Measures In Connection With Russias Actions Destabilizing The Situation In Ukraine And Art. 7 Section 1 Of The Act On Special Solutions For Counteracting Support For Aggression Against Ukraine And For The Protection Of National Security, Submitted Pursuant To Art. 125 Section 1 Of The Public Procurement Law, In Accordance With The Template Constituting Annex No. 10 A To The Swz (Signed With A Qualified Electronic Signature).
