Part I Provision Of Services Consisting In Providing A Temporary Place In A Shelter For Homeless People - Men Referred By The Municipal Social Welfare Center In Wołów In Accordance With The Provisions Of Art. 48A Of The Act On Social Assistance Of March 12, 2004. ➢ The Subject Of The Order Is To Provide 24-Hour Temporary Shelter Along With Daily Meals And Ensuring The Necessary Social Conditions And Services Aimed At Strengthening Social Activity, Solving Homelessness And Gaining Independence In Life To Homeless People Referred By The Municipal Social Welfare Center. ➢ Service Provision Period: From May 1, 2026 To December 31, 2026, 7 Days A Week. ➢ The Contractor Is Obliged To Indicate In The Offer The Place Where The Service Will Be Provided. The Ordering Party Reserves The Right To Verify The Indicated Place In Terms Of Preparation For The Implementation Of The Ordered Service Before Concluding The Contract And Throughout The Entire Order Execution Period. ➢ The Expected Number Of Homeless People Covered By The Service Is 3 People. Due To The Specificity Of The Order, Which Is Characterized By Variability Of Customer Needs Due To Their Changing Health Condition And Personal Situation, The Number Of Homeless People Provided By The Ordering Party Is The Number Expected Throughout The Duration Of This Order. The Ordering Party Reserves The Right To Reduce Or Increase The Above. The Number Of People Depending On The Actual Needs In This Respect, Subject To The Provisions Specified In The Contract Template (Appendix No. 4 To The Swz). ➢ The Contractor Will Receive Remuneration For The Actual Number Of People Referred By The Municipal Social Welfare Center In Wołów. ➢ Admission Of A Homeless Person To The Shelter Will Take Place After Prior Arrangement Of The Date, Based On An Administrative Decision Issued By The Municipal Social Welfare Center In Wołów. ➢ The Contractor Is Obliged To Immediately Inform The Ordering Party About Any Change In The Life And Health Situation Of The Person Using The Shelter. ➢ The Ordering Party Stipulates That The Services May Only Be Provided By Entities Running Shelters For Homeless People Entered By The Relevant Voivode On The List Of Establishments Providing Temporary Shelter. ➢ The Standard Of Basic Services Provided In A Shelter For Homeless People And The Standard Of The Facility In Which The Shelter For Homeless People Is Located Must Be Consistent With Annex No. 2 To The Regulation Of The Minister Of Family, Labor And Social Policy Of April 27, 2018 On Minimum Standards For Night Shelters, Shelters For Homeless People, Shelters For Homeless People With Care Services And Heating Rooms.
