5.1 Name Of The Order: Reconstruction Of Ul. Tourist In Sułkowice-Łęg. 5.2 The Subject Of The Order Includes In Particular: Reconstruction Of Ul. Turystyczna In Sułkowice-Łęg - Connector Between Ul. Beskidzka And Ul. Central, Including The Construction Of Lighting And Elimination Of Conflicts With The Overhead Telecommunications Network. 5.3 The Description Of The Subject Of The Order Is Included In The Annexes To The Swz, I.E.: Annex No. 3 And 3.1 Design Documentation, 3.2 Technical Specification For The Execution And Acceptance Of Works, 3.3 Bill Of Quantities. The Contractor Is Obliged To Complete The Order On The Terms And Conditions Described In The Draft Contract. 5.4 The Ordering Party Requires Pursuant To Art. 95 Section 1 Of The Public Procurement Law, Employment Under An Employment Contract In Ones Own Enterprise Or By A Subcontractor Of Persons Performing Activities Specified By The Ordering Party In The Scope Of Contract Execution, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974, Labor Code, I.E.: Physical Work On The Construction Site, Including: Preparatory And Demolition Works, Earthworks, Felling And Planting Trees, Asphalt Concrete Road Base And Surface, Exit And Sidewalk Base And Surface, Roadside Construction, Marking And Finishing Works, Construction Of Lighting, Demolition And Construction Of A Telecommunications Network. 5.4.1 The Contractor Will Be Obliged To Submit To The Ordering Party, Upon Each Request And Within The Deadline Specified In The Request, The Following Evidence Confirming The Fulfillment Of The Requirement Of Employment Under An Employment Contract By The Contractor Or Subcontractor Of Persons Performing The Indicated Activities During The Execution Of The Order: A) Declaration Of The Contractor Or Subcontractor On The Employment Under An Employment Contract Of Persons Performing The Activities Covered By The Request Of The Ordering Party.
