1. The Subject Of This Order Is The Design And Execution Of Construction Works Consisting In Waterproofing The Building Of The Department And Institute Of Forensic Medicine Of The Medical University Of Warsaw At Ul. Oczki 1 In Warsaw, Together With The Elimination Of The Effects Of Moisture In Basement Rooms, On The Terms Specified In The Draft Agreement, Constituting Annex No. 6 To Swz.2. The Detailed Scope Of Design Work And Construction Works Was Specified In The Functional And Utility Program, Containing The Technical Expertise, Constituting Annex No. 2 To Swz.3. Usługi Projektowe I Wykonanie Robót Budowlanych, Stanowiących Przedmiot Zamówienia Wykonawca Wykona, W Następujących Etapach:1)Etap I – Opracowanie Dokumentacji Projektowo-Kosztorysowej Dla Zadania Inwestycyjnego Pn. Design And Execution Of Construction Works In The Building Of The Department And Institute Of Forensic Medicine Of The Medical University Of Warsaw At Ul. Oczki 1 In Warsaw, Together With The Elimination Of The Effects Of Moisture In Basement Rooms, In Accordance With The Assumptions Included In The Functional And Utility Program, Constituting Annex No. 2 To The Agreement. As Part Of This Stage, The Contractor Will Obtain A Valid Decision Enabling The Implementation Of Construction Works, 2) Stage Ii - Execution Of Construction Works, Based On The Developed Design And Cost Estimate Documentation Referred To In Point 1, Together With The Performance Of The Authors Supervision Throughout The Entire Period Of Execution Of Construction Works, - In Compliance With The Deadlines Specified In Chapter Iv Of The Swz And In § 3 Of The Draft Agreement.4. Based On Art. 131 Section 2 Of The Act, The Ordering Party Provides For The Possibility Of Carrying Out An On-Site Visit After Reporting Such A Request. An Application For A Site Visit Must Be Submitted No Later Than April 1, 2026, On The Platform. 5. The Ordering Party Also Informs That An On-Site Visit Is Not Obligatory. In The Light Of The Above, The Offer Of A Contractor Who Will Not Participate In Such A Visit Will Not Be Subject To Rejection, Pursuant To Art. 226 Section 1 Point 18 Of The Act.6. The Ordering Party Requires That In Accordance With Art. 95 Of The Act, Activities Involving Pressure Injection Of Walls, Plastering And Painting Of Walls, Unless Performed By Persons As Part Of Their Business Activity, Were Performed By Employees Employed By The Contractor Or Subcontractors Under An Employment Contract Within The Meaning Of Art. 22 § 1 Of The Act Of June 26, 1974, Labor Code (Consolidated Text: Journal Of Laws Of 2023, Item 1465, As Amended). 7. Pursuant To § 2 Of The Draft Agreement, The Contractor Is Obliged To Conclude, At Its Own Expense, A Contract Or Subsequent Insurance Contracts Establishing Insurance Protection Against Any Property Damage, In Which The Ordering Party Will Be Co-Insured, Which May Occur In Connection With Random Events On The Site Of Construction And Assembly Works, In Particular Those Involv
