Eliminating Health Risk Factors At Work - Individual Support For Employees Of The District Office In Radomsko: - Individual Support From A Psychologist - 360 H; - Support From A Physiotherapist - 450 H; - Physiotherapeutic Consultations - 200 H; - Employee Examinations Along With Consultation Of Results And Health Education - 114 Sets. Specific Types Of Support Are Described In Detail In Annex No. 9 To The Swz - Description Of The Subject Of The Contract In Table No. 1 Types Of Support For Employees.Additional Arrangements:1. Health Services Must Be Provided By Persons Or Entities Entered In The Register Of Entities Performing Medical Activities (Rpwdl). The Requirement Does Not Apply To A Psychologist. 2. Place Of Service, Offices Of The Entity Providing The Service In The City Of Radomsko.3. In Relation To The Number Of Hours Of Particular Types Of Support, Clock Hours Should Be Understood.4. It Is Planned To Establish Flexible Dates And Hours Of Support.5. All Services Regarding Specific Types Of Support Will Be Provided In Accordance With The Principles Of Equal Opportunities And Non-Discrimination For People With Disabilities, Using Language Sensitive To Gender And Social Inequalities.6. In Order To Ensure Equal Opportunities For Women And Men, It Is Planned To Establish Flexible Dates And Hours Of Support, Especially To Enable Women To Reconcile Professional And Family Responsibilities. Specialists Must Use Equality Slogans, Easy And Gender-Sensitive Language, And The Image Of Active Women.7. The Contractor Will Be Obliged To Submit A Schedule Of Individual Types Of Support In Electronic And Paper Versions Within 7 Calendar Days From The Date Of Signing The Contract. The Schedule Will Be Subject To Acceptance By The Ordering Party.8. The Contractor Will Be Obliged To Submit Confirmation Of Employees Participation In Individual Forms Of Individual Support (I.E. Psychologists Support, Physiotherapists Support, Employee Examinations With Consultation) In A Paper Version By The 5Th Day Of Each Subsequent Month After Concluding The Contract, Except For The Month Of June Where Confirmation Of Employees Participation Should Be Submitted No Later Than June 29, 2026. 9. The Contractor Will Be Obliged To Keep Secret All Information Obtained During The Performance Of The Contract And To Use It Only For Purposes Related To The Implementation Of The Contract.10. The Contractor Will Be Obliged To Apply And Fulfill Information And Promotional Obligations Ordered By The Ordering Party.11. The Contractor Will Be Obliged To Provide The Service In Accordance With The Dnsh Principle, In Particular: - Organizing Meetings Taking Into Account The Conditions Regarding The Place And Dates Enabling Access By Public Transport, If Possible, With A Place To Attach Bicycles; - Using Ecological Hygiene/Disinfectants; - Segregating Medical Waste.12.
