The Subject Of The Order Is Purchase, Delivery And Unloading Of Medicines For The Substitution Treatment Program To The Hospital Pharmacy Spec. Psychiatric Zoz In Łódź. In Accordance With The Detailed Opz, Constituting Annex No. 2 To Swz.2. The Subject Of The Order Delivered By The Contractor Will Be Brand New, Ready For Use, Free From Physical And Legal Defects, Originally Packed, Of The Highest Quality And Will Have A Validity Period Of At Least 12 Months From The Date Of Delivery, And The Expiry Date Must Be Legible. In The Case Of A Product Or Preparation Whose Expiry Date Given By The Manufacturer Is 12 Months Or Less, It Is Allowed To Deliver Goods Whose Expiry Date Will Be 2/3 Of The Expiry Date Given By The Manufacturer. 3. Given By Order Quantities Of The Products Specified In The Product And Price Form, Which Are The Subject Of This Public Procurement Procedure, Are Estimated Values Based On The Current Demand Of The Order. And During The Implementation Of The Contract In Question May Change Up To 20% Of The Given Quantities (Reduce Or Increase) Depending On The Current Needs Of The Ordering Party. 4. Pharmaceutical Products Should Meet The Conditions For Admission To Trading And Use In The Territory Of The Republic Of Poland In Accordance With The Provisions Of The Act Of April 7, 2022 On Medical Devices - If Applicable And The Act Of September 6, 2001 Law Pharmaceutical. 5. The Order Accepts 1 Piece Or 1 Package As The Unit Of Measurement. If The Number Of Pieces In The Package Is Not Specified, The Ordering Party Will Compare The Price Of The Offer For One Piece. 6. The Order Will Order Packages Not Smaller Than Those Specified In The Assortment And Price Form (Appendix No. 2 To Swz). 7. If: 7.1. Only Smaller Packages With A Different Number Of Pieces Are Available On The Market, Then The Contractor Should Recalculate The Offered Quantities Accordingly. With The Demand For The Required Total Quantity By The Ordering Party And Rounding Up Any Fractional Quantities To A Full Package.7.2. There Are Smaller Packages Available On The Market With A Different Number Of Pieces And Packages With A Number Of Pieces Consistent With The Orders Demand Or In Another Form, The Contractor Should Recalculate The Offered Quantity Accordingly In Accordance With The Demand For The Required Total Quantity By The Orderer, Rounding It Up To A Full Package.7.3. There Are Packages Available On The Market In Terms Of Quantity Consistent With The Orderers Demand And Larger, And The Contractor Proposes A Larger Package, Then The Contractor Should Recalculate The Offered Quantity According To The Demand For The Required Total Quantity By Order, Rounding Up Any Fractional Quantities To The Full Package. 7.4. If, When Converting From Pieces To Packages, The Number Of Packages Is Not An Integer, The Full Number Of Packages Should Be Provided, Rounded Up. The Order Does Not Allow The Conversion Of The Number Of Packages To Two Decimal Places.7.5. In The Event Of Converting The Required Quantities
