Quotations must be accompanied by thefollowing (Failure to attach will lead to disqualification): o Pin/CSD number to be completed in MBD 1 on the document o Copyof CK/Company registration certificate, o Certified copy of I.D of Directors (Certificationmust not be older than 03 months) o Bidder (Company and director/partner or sole propriety) must attach proof of municipal rates on municipality letterhead not older than 90 days. If the statement of municipal rates is not in the name of the bidder an affidavit from SAPS must be attached.
