Medical Consumables – Accessories For Corpuls Defibrillators. The Subject Of This Public Contract Is The Conclusion Of A Framework Purchase Agreement (Hereinafter Also Referred To As The Contract) With One Supplier For The Supply Of Accessories And Consumables (Hereinafter Referred To As Goods) For The Corpuls3 And Corpuls3 Slim Defibrillators, Which Are Currently Used By The Contracting Authority In The Provision Of Pre-Hospital Emergency Care. More Detailed Information On The Subject Of Public Procurement Is Given In Appendix No. 4 Zd. As This Is A Medical Device For Which The Client Requires Consumables And Accessories To Be Supplied, It Is Necessary To Supply Only New Goods That Ensure Full Compatibility With The Existing Corpuls3 And Corpuls3 Slim Device. The Supplier Shall Indicate In Appendix No. 5 Zd The Designation (Name) Of The Individual Items Of Goods, Which Will Subsequently Be Indicated For Better Identification Of The Goods In The Partial Order. If The Technical Specification Contains References To Individual Trade Names, Special Designations Of Companies Or Products, Useful Types, Performances Or Business Materials That Are Typical For A Certain Company Or Organizational Unit, The Client Allows The Use Of Other, Technical And Qualitatively Identical Or Better Solutions, Which, However, Must Fully Meet The Clients Requirements For Guaranteed Compatibility With The Existing Corpuls3 And Corpuls3 Slim Device And The Clients Equipment, For Which The Consumables And Accessories For The Device Are Intended. If The Supplier Offers Another Product (Goods) That Meets The Above-Mentioned Compatibility With The Corpuls3 And Corpuls3 Slim Device, The Supplier Will State This Fact (Indicating The Replacement - ... And The Designation Of The Corresponding Product) In Brackets After The Required Sub-Item In Appendix No. 4 Zd And In Appendix No. 5 Zd. The Requirements For The Specified Minimum Technical Parameters Must Be Met By The Offered Goods. The Client Has The Right To Subsequently Request From The Supplier The Presentation Of This Offered Replacement Product - A Sample Of A Specific Product, To Verify Compatibility With The Corpuls3 And Corpuls3 Slim Devices. On The Basis Of Regulation (Eu) 2017/745 Of The European Parliament And Of The Council Of April 5, 2017 On Medical Devices, The Contracting Authority Requires That, As Part Of The Subject Of Performance, The Delivery Of A Declaration Of Conformity To The Affected Supplies Of Consumables (The Declaration Will Be Part Of The Tax Document/Invoice Or Delivery Note). Part Of The Delivery Of The Subject Of Performance Are Instructions For Use For Supplies Of Consumables In The Czech Language.
