Tenders Are Invited For Providing Services Provided In Veterinary Offices Consisting Of Vaccination Against Rabies Along With Microchip Marking In Accordance With The Iso Standard For Owner Dogs And Cats

1. The Subject Of The Order Is The Provision Of A Service Consisting In Promoting Castration And Microchipping Campaigns By Performing Vaccinations Against Rabies In Offices (Estimated Quantity 200 Pieces) Along With Microchip Marking (Estimated Quantity 200 Pieces) In Accordance With The Iso Standard Of Dogs And Cats Owned By The Residents Of The City And Commune Of Góra Kalwaria.2. If Your Pet Does Not Have A Microchip, You Must Have Your Pet Microchipped. The Condition For Vaccination Under This Agreement Is That The Animal Be Microchipped.3. The Contractor Is Obliged To: 1) Inform Animal Owners About The Advantages Of Microchipping, Sterilization, Castration And Vaccination Of Animals, 2) Provide Free Veterinary Advice On Microchipping, Sterilization, Castration And Vaccination Of Animals To Persons Interested In Using The Veterinary Service Covered By This Agreement, 3) Place In A Visible Place On The Premises Of The Veterinary Facility Information Regarding The Veterinary Service Covered By This Agreement Performed At The Expense Of The Góra Commune Calvary.4. The Contractor Will Perform The Services Specified In Section 1 And Section 2, In Accordance With Applicable Law, Good Veterinary Practices, Own Knowledge And Experience, In A Way That Minimizes Animal Suffering.5. Services Specified In Section 1 And 2 Must Be Performed In A Veterinary Facility That Meets The Requirements Of Applicable Law.6. The Contractor Will Provide Appropriate Pharmacological Agents, Equipment And Conditions For The Proper Performance Of The Subject Of The Contract.7. Provision Of Services Specified In Section 1 And 2 Only By A Veterinarian Who Has The Active Right To Practice The Profession In Accordance With The Act Of December 21, 1990 On The Profession Of Veterinarian And Veterinary Chambers (Consolidated Text: Journal Of Laws Of 2026, Item 125, As Amended) Or Who Has The Ability To Provide Services In Accordance With Art. 2H Of The Given Act.8. Before Commencing Vaccination, The Contractor Is Obliged To: 1) Verify Whether The Animal To Be Vaccinated Has Already Been Microchipped, If Not - Inform The Animals Owner That The Free Vaccination Requires The Animal To Be Microchipped; 2) Perform A Preliminary Examination To Determine Whether The Animals Health Condition Allows The Service To Be Performed; 3) Post About The Provision Of The Service With The Date, Signature And Stamp Of The Veterinarian; 4) Check Whether The Microchip Implanted In The Animal Is Functional And Registered;5) If An Animal Is Marked With A Microchip, Including The Number Of The Microchip With Which The Animal Was Marked In The Health Booklet.9. The Contractor Is Obliged To Have A Working Microchip Reader Throughout The Entire Period Of Service Provision.10. After Carrying Out The Procedure, The Contractor Will Check Whether The Microchip With Which The Animal Was Marked Is Active And Whether The Number Of The Microchip With Which The Animal Was Marked Agrees With The Number On The Applic
