The Subject Of The Order Is A Service Consisting In The Collection, Transport And Management Of Stabilized Municipal Sewage Sludge (Waste Code 19 08 05) From The Sewage Treatment Plant In Kolbuszowa Dolna. The Amount Of Waste Expected By The Ordering Party To Be Generated During The Duration Of The Contract: Sludge With Code 19 08 05 - Approximately 1,500 Mg. Detailed Description Of The Subject Of The Order: 1. The Contractor Should Have A Permit To Collect And Process Waste In Accordance With Art. 41 Of The Waste Act Of December 14, 2012 (Journal Of Laws Of 2023, Item 1583), With Particular Emphasis On Waste Code 19 08 05 And The Declared Recovery Method R3. 2. The Contractor Should Have An Entry In The Register (Bdo Registration Number) Kept By The Voivodeship Marshal, Which Allows Him To Conduct Business Involving The Transport Of Waste In Accordance With Art. 50 Section 1 Point 5 Letter B Of The Waste Act In The Scope Of Waste Code 19 08 05. 3. The Contractor Must Have A Current And Final Decision Of The Minister Of Agriculture And Rural Development Authorizing The Placing Of Processed Waste On The Market As A Product (Fertilizer Or Agent Supporting Plant Cultivation) In Accordance With The Act On Fertilizers And Fertilization. 4. Waste With Code 19 08 05 (Stabilized Municipal Sewage Sludge) Is Collected At The Treatment Plant In The Place Designated For Its Storage. 5. The Waste Generator Will Use Its Own Equipment (Loader) To Load Waste Code 19 08 05 Onto The Means Of Transport Provided By The Contractor. 6. The Contractor Is Obliged To Have At Least 2 Containers Adapted To Transport Municipal Sewage Sludge With Maximum External Dimensions: Length 580 Cm, Width 245 Cm, Height 174 Cm. 7. The Contractor Assumes Responsibility And Is Obliged To Manage Waste In A Manner Consistent With The Principles Set Out In The Waste Act, Environmental Protection Requirements And In Accordance With The R3 Recovery Process, Excluding The Use Of Sludge Pursuant To Art. 96 Section 1 Points 1-5 Of The Waste Act (I.E. Excluding Recultivation And Direct Use In Agriculture). 8. Upon Transfer Of Waste, The Contractor Shall Bear Full Legal And Financial Responsibility For Collection, Transport And Management, In Accordance With Art. 27 Of The Waste Act. 9. Received Waste With Code 19 08 05 Requires Confirmation By The Contractor On The Waste Transfer Card (Kpo) Prepared By The Ordering Party In The Bdo System. 10. In The Event Of Inspections By State Inspection Services (Including The Provincial Inspectorate Of Environmental Protection And The Plant Health And Seed Inspection), As A Result Of Which Penalties Are Imposed On The Ordering Party Resulting From Waste Management That Is Inconsistent With The Law, The Certificate Of The Ministry Of Agriculture And Rural Development Or The Contract, The Contractor Is Obliged To Refund The Equivalent Of These Penalties. 11. Waste Collection And Disposal Will Be Carried Out By Our Own Means Of Transport, From: From 7:00 A.M.
