The Subject Of The Contract Is The Implementation Of A Task Consisting In Relieving Family Members Or Guardians Of People With Disabilities By Supporting Them In Their Daily Duties By Providing Temporary Replacement. The Implementation Of The Task Is Co-Financed By The Solidarity Fund. The Order Should Be Performed In Accordance With The Rules Set Out In The Program Of The Minister Of Family, Labor And Social Policy Respite Care For Local Government Units - 2026 Edition (Hereinafter Referred To As The Programme), Which Constitutes Annex No. 7 To The Swz. Respite Care Services Will Be Provided To Residents Of Ruda Śląska - Family Members Or Guardians Providing Direct Care For Disabled People Living In The Same Household, Including Children (Hereinafter Referred To As Program Participants), And Provided To Their Wards Indicated To The Contractor By The Ordering Party. The Services Will Be Provided In The Form Of A Daily Stay At The Place Of Residence Of The Disabled Person. Expected, Estimated Number Of People Using The Services: 11 (3 Children Between 2 And 16 Years Of Age And 8 People With Severe Disability Certificates Or Equivalent Certificates). This Number May Increase Or Decrease During The Implementation Of The Contract. Respite Care Services Will Be Provided As Part Of The Day Stay.
