Health And Social Work Services – Providing Health Services In The Form Of Tests And Possible Vaccinations. 1) The Subject Of The Order Is To Perform Medical Examinations, Diagnostic Tests, Specialist Consultations And Protective Vaccinations For Officers And Employees Of The Małopolska Police Garrison, The Management Board In Krakow Of The Cbśp, The Department In Krakow Of The Bswp And The Department In Krakow Of The Cbzc, Assigned To Serve In Police Contingents Abroad For The Years 2026-2028. 2) The Ordering Party Requires The Provision Of Services In The City Of Kraków. 3) The Ordering Party Expects 3 Departures (44 People In Total) And 3 Returns (46 People In Total). 4) A Detailed Description Of The Subject Of The Order And The Expected Quantities Of Services Are Included In Annex No. 1. The Service Will Be Provided Depending On The Needs Of The Ordering Party. The Expected Amounts Of Medical Services Given In Annex 1 Are Forecast Values. These Values Are Indicated Only For The Purpose Of Selecting The Most Advantageous Offer By The Ordering Party And Do Not Constitute An Obligation Of The Ordering Party Towards The Contractor. At The Same Time, The Ordering Party Guarantees The Performance Of The Contract At A Minimum Level. 70% Of The Contract Value. 5) The Ordering Party Requires That The Examination, Qualification For Vaccination And Vaccination With A Single Dose Be Carried Out Within One Day Each. 6) The Basis For The Provision Of Services That Are The Subject Of The Contract Will Be Personal Referrals Issued By The Ordering Party According To Template No. 2 (Appendix No. 2 To The Contract). 7) The Ordering Party Requires That The Doctor Performing The Examination, In Addition To Preparing Appropriate Medical Documentation, Issue A Certificate Of Medical Certificate In Polish And English According To The Template Constituting Annex 3 To The Contract. 8) The Next Planned Departure Date Of The Officers Is June 2026. Tests And Vaccinations Should Be Carried Out Before Departure, But They Cannot Take Place In The Period From April 20 To May 20, 2026, Due To The Officers Official Obligations.
