The Subject Of The Order Is An Investment Consisting In The Purchase And Construction Of A Container Building For The Needs Of: A Surgical And Neurological Clinic And For Night And Holiday Medical Care, As Well As The Modernization Of A Gynecological Clinic On The Premises Of The Multi-Specialty District Hospital Sa, Located At Ul. Pyskowicka 47-51 In Tarnowskie Góry.2. The Task Will Be Implemented In The Design And Build Formula.3. The Subject Of The Order Is:• Preparation Of Construction And Installation Designs,• Obtaining A Valid Building Permit,• Delivery And Assembly Of A Modular Building With The Necessary Technical Infrastructure And Land Development,• Modernization Of Rooms In The Hospital Building,• Delivery And Installation Of An Elevator Platform For Disabled People Along With The Construction Of Accesses And Ramps• Bringing The Facility To A Condition Suitable For Use.4. The Planned Investment Is Aimed At: • Providing Medical Care In The Neurological And Surgical Outpatient Clinic, • Providing Medical Care Services In The Gynecological Outpatient Clinic (The Existing Part Of The Hospital Building Subject To Modernization), • Ensuring Communication For Disabled People Using Wheelchairs (Platform For Disabled People, Roads And Access), • Adapting The Hospital Facility To The Possibility Of Providing Night And Holiday Medical Care.5 All Necessary Information Has Been Included In The Functional And Utility Program, Constituting Annex No. 2 To The Terms Of Reference.
