The Subject Of The Order Is The Provision Of Respite Care Services At The Place Of Residence Of A Person With A Disability Under The Departmental Program Of The Minister Of Family, Labor And Social Policy Respite Care - 2026 Edition. 2. The Services Will Be Provided For A Maximum Of 2,640 Hours. The Planned Number Of Hours Of Providing Respite Care Services Is 240 Hours Of Services For 1 Person With A Disability. In The Event Of A Large Number Of Applications, The Hours Will Be Divided Among The Applicants.3. Due To The Specificity Of The Subject Of The Order, The Ordering Party Reserves The Right To Change The Number Of People Using Care Services During The Term Of The Contract.4. The Ordering Party Guarantees The Minimum Scope Of The Order: Care For 8 People With Disabilities, I.E. A Minimum Of 1,920 Hours Of Respite Care Services.5. A Detailed Description Of The Conditions For Implementing The Subject Of The Order Is Included In The Draft Provisions Of The Contract Constituting Annex No. 8 To Swz.6. The Contractor Is Obliged, Throughout The Period For Which The Agreement Is Concluded, To Have Civil Liability Insurance For The Activities Related To The Subject Of The Order For A Guarantee Amount Of Not Less Than Pln 70,000.00 (In Words: Seventy Thousand Zlotys 00/100), Valid No Later Than From The Date Of Signing The Contract Until The Final Acceptance. The Ordering Party Will Accept The Liability Insurance Document Held By The Contractor In The Scope Of Activities Related To The Subject Of The Order For The Insured Sum Required By The Ordering Party, Which Will Be Concluded For An Annual Period And Will Be Renewable While Maintaining Continuity Of Insurance Coverage Until Final Acceptance. In Such A Situation, The Contractor Is Obliged To Provide The Ordering Party With A Valid Third Party Liability Insurance Document For The Activities Related To The Subject Of The Order No Later Than Within 7 Days From The Date Of Issuance Of This Document.7. Before Commencing The Implementation Of The Contract, The Contractor Will Be Obliged To Provide The Ordering Party With A Certificate Of No Criminal Record From The National Criminal Register Or The Criminal Register Of Another Country And Information About Not Being Included In The Register Of Perpetrators Of Sexual Offenses In The Form Of A Printout Of Information Downloaded From The Register For At Least One Guardian Of A Person With A Disability, As Well As, At The Request Of The Ordering Party, Documenting The Experience Of Persons Indicated In Annex No. 9 Of The Swz Or Submitting Documents Confirming Obtaining Qualifications In The Profession Assistant To A Disabled Person, Nurse, Pck Sister, Caregiver For An Elderly Person, Medical Caregiver, Educator, Psychologist, Occupational Therapist, Physiotherapist Or Other Professions And Specialties Of A Medical Or Caring Nature.8. The Ordering Party Requires The Contractor Or Subcontractor To Employ, On The Basis Of An Employment Relationship, Person
