Cpv 85121270-6 Psychiatric And Psychological Services1. The Subject Of The Order Is To Conduct And Prepare Written In-Depth Diagnoses For Children And Adolescents Staying In Foster Care In The Koło District. The Childs Developmental Status And Deficits Will Be Assessed. The Diagnosis Will Indicate The Potential Necessary To Correct Them And Proceed Further.2. As Part Of The Task, The Contractor Will Provide 12 Hours Of Service Consisting In Conducting A Comprehensive, In-Depth Diagnosis In Accordance With The Indications Resulting From The General Development Diagnosis For 3 Pupils Of Family Foster Care From The Koło District. 3. As Part Of The Diagnosis, The Contractor: 1) Conducts An Interview With The Childs Guardians And The Child - In Order To Identify The Needs And Occurring Difficulties. The Interview Includes A Conversation About The Childs Educational Environment And His/Her Functioning In It; 2) Gets Acquainted With The Opinions Of Other Specialists; Conducts Direct Observations Of The Childs Behavior In The Office;3) Carries Out Necessary Tests In Accordance With Applicable Rules Regarding Them;4) Discusses The Test Results With The Childs Guardians And, If Necessary, With The Children; 5) Prepares A Written Diagnosis.4. The Contractor Undertakes To Carry Out An In-Depth Diagnosis By A Diagnostic Team Consisting Of At Least The Following Specialists: 1) Psychologist: - Completed Uniform Masters Studies Or First- And Second-Cycle Studies In The Field Of Psychology - Minimum 2 Years Of Experience In Working With Children And Adolescents 2) Speech Therapist: - Completed Uniform Masters Studies Or First- Or Second-Cycle Studies In The Field Of Speech Therapy Or Masters Studies Or First- And Second-Cycle Studies In The Field Of Pedagogy/Psychology Supplemented With Studies Postgraduate Studies In Speech Therapy/Neurological Speech Therapy, - Minimum 2 Years Of Experience In Working With Children And Adolescents.3) Sensory Integration Diagnostician: - Completed Studies In The Field Of Physiotherapy, Psychology Or Pedagogy And Completed At Least 2Nd Level Of A Sensory Integration Course Or Postgraduate Studies In The Field Of Sensory Integration, - Minimum 2 Years Of Experience In Working With Children And Adolescents.3. The Service Will Be Performed In The Koło District, In The Place Indicated By The Contractor.4. Other Rules For Implementing The Task Are Included In The Description Common To All Parts.
