The Subject Of The Order Is Provision Of Comprehensive Medical Services (Medical Packages) Within The Scope Of Group Health Insurance For Employees Of Metro Warszawskie Sp. Z O. O. And Their Family Members (Spouse/Partner, Child). 2. A Detailed Description Of The Subject Of The Order Is Included In The Technical Specification With Annexes (Hereinafter Referred To As Ts), Constituting Annex No. 1 To The Swz. 3. The Subject Of The Order Will Be Completed In Accordance With The Procedure Documentation, I.E. This Specification Of Order Conditions (Hereinafter: Swz) And Its Annexes, In Particular The St And The Contract Template Constituting Annex No. 3 To The Swz. 4. The Subject Of The Order Includes The Provision Of Group Health Insurance Services As Part Of The Following Packages: 1) Basic Individual Employee Package (Variant I - Financed By The Ordering Party); 2) Optional Individual Employee Package (Variants Ii, Iii, Iv - Co-Financed By The Ordering Partys Employees On A Voluntary Basis); 3) An Optional Package For Family Members Of The Ordering Partys Employees (Variants I, Ii, Iii, Iv - Financed By The Ordering Partys Employees On A Voluntary Basis). 5. Individual Insurance Packages Will Consist Of Four Variants. The Scope Of Individual Insurance Variants Is Included In Annex No. 1 To The St - Minimum Scope Of Benefits In Individual Variants. 6. The Ordering Party Provides For The Possibility Of Using The Option Referred To In Art. 441 Section 1 Of The Public Procurement Law, Taking Into Account The Following Principles: 1) The Optional Scope Constituting The Subject Of The Order Is Specified In Point 4.4 Of The Swz; 2) The Optional Scope Of The Subject Of The Order Will Be Implemented Only After The Ordering Party Has Informed The Contractor In Writing About The Need To Use The Option; 3) An Option Is An Entitlement Of The Ordering Party, Which The Ordering Party May, But Does Not Have To, Use As Part Of The Implementation Of The Subject Of The Order; 4) If The Ordering Party Does Not Use The Option, The Contractor Is Not Entitled To Any Claims In This Respect; 5) Detailed Provisions Regarding The Ordering Partys Exercise Of The Option Right Are Specified In The Contract Template (Appendix No. 3 To The Swz). 7. The Ordering Party Requires Employment Under An Employment Contract Within The Meaning Of The Provisions Of The Act Of June 26, 1974 - Labor Code (Consolidated Text: Journal Of Laws Of 2025, Item 277, As Amended) By The Contractor Or Subcontractor Of Persons Performing The Following Activities During The Implementation Of The Subject Of The Order: 1) Providing Hotline Service (Providing Teleconsultations), 2) Recording Remuneration Receipts For Medical Services Provided, 3) Performing The Activities Of An Administrative Guardian And A Business Guardian. A Detailed Description Of How To Verify Compliance With This Requirement Is Provided In The Content Of The Contract Template (Appendix No. 3 To The Swz). 8.
