I. Subject Of The Order The Subject Of The Order Is The Provision Of Laundry Services For The Provincial Hospital. Zofia Née Zamoyska Tarnowska In Tarnobrzeg. Ii. Description Of The Subject Of The Contract 1. Cpv Code According To The Common Procurement Vocabulary:98310000-9 Laundry And Dry Cleaning Services98311000-6 Laundry Collection Services 98315000-4 Ironing Services 2. A Detailed Description Of The Subject Of The Contract Is Included In Annex No. 2 To The Swz - Price Form. 3. The Provision Of Laundry Services Includes: Washing Hospital Linen, Mops, Mattresses, Mattress Covers, Latex Foam Mattresses, Blankets, Pillows, Curtains, Etc., As Well As Chemical And Thermal Disinfection, Ironing, Minor Repairs (Stitching, Darning, Sewing On Buttons, Strings), Sewing In Damaged Zippers, Ironing And Dry Cleaning Of Products Not Suitable For Water Washing. Along With Transport. Dry Cleaning Covers In Particular The Following Products: Plastic And Canvas Blinds, Nurses Capes, Velvet And Velvet Curtains. 4. Before Concluding The Contract And For The Duration Of The Contract, The Ordering Party Has The Right To Verify The Information Provided In The Offer And To Control The Conditions In Which The Service Is Provided By Visiting The Contractors Laundry. 5. Determination Of Non-Compliance Of The Information Provided In The Offer By The Contractor And The Results Of The Inspection May Constitute Grounds For Rejecting The Offer Or Withdrawing From The Contract With The Ordering Partys Demand For Compensation. 6. The Rules For The Implementation Of The Service In Question Are Specified In The Draft Agreement Constituting Annex No. 5 To The Swz. 7. Laundry Requirements. 1) A Laundry Room For Washing Hospital Linen Should: A) Have A Hygienic Barrier Between The Dirty And Clean Sides By Physical And Functional Division Into Dirty And Clean Zones In The Manner Referred To In Point 3; B) Should Use Technology Adapted To Washing Hospital Linen; 2) Laundry Rooms Should Be Arranged In A Way That Ensures Maintaining A Hygienic Barrier, Understood As Complete Elimination Of Contact Between Clean And Dirty Linen And Employees From These Two Zones; 3) A Laundry Room With A Hygienic Barrier Should Have: A) A Dirty Zone - Which Includes Rooms For Receiving And Folding Dirty Laundry, Rinsing Diapers, Loading Laundry Into Laundry Machines, Rooms For Storing Washing Agents, Preparing Washing Solutions And Toilets; B) Clean Zone - Which Includes Rooms For Unloading Linen From Laundry Machines, Drying, Ironing And Sterilizing Linen For Newborns And Younger Children, Washing And Disinfecting Strollers (Clean Side), A Room For Storing Washed And Disinfected Materials, A Linen Repair Room, An Issuance Room, Administration And Social Support Rooms And Toilets; C) A Three-Stage Cloakroom Lock Located Between The Clean And Dirty Zones, Separating Both Areas With Walls Up To The Full Height Of The Room. 4) Laundry Equipment Should Be Built Into The Wall Dividing The Laundry Room Into
