Provision Of Assistant Services By 1 Assistant As Part Of The Project Home Telecare Program - Support Program For Senior Policy And People With Disabilities For 1 Person With A Significant Or Moderate Degree Of Disability Aged 18 And Over And/Or With Intellectual Disability Aged 18 And Over. The Assistant Service Includes Supporting People With Disabilities In Performing Basic Everyday Activities Necessary For The Active Social, Professional And Educational Functioning Of People With Disabilities. Assistant Services Include Help And Support For The Project Participant In Everyday Activities, Such As:1. Support In Moving, Among Others To The Doctor, To Service Points And Other Public Places And Assistance In These Places;2. Spending Free Time, Including Support During Cultural, Sports And Recreational Activities, E.G. Sign Language Translation;3. Support In Moving To Schools And Educational Institutions;4. Support During Educational Activities And In The Learning Process;5. Activities That Facilitate Communication With The Environment In The Case Of Deaf People, Or Assistance To A Physically Disabled Person In Activities That Are Impossible To Perform Independently At The Workplace Or While Looking For A Job. Assistant Services That Are The Subject Of The Order Will Be Performed On Working Days, As Well As On Saturdays, Sundays And Holidays From 6.00 A.M. To 10.00 P.M. One Hour Of Assistant Service Is Assumed To Be 60 Minutes. The Deadline For Completing The Order Is 21 Months From The Date Of Signing The Contract, But No Longer Than December 31, 2027. Planned Number Of Hours Within Individual Parts: In Tasks 1-4, One Person With A Disability For Each Task, On Average 60 Hours Per Month Per Person - 1260 Hours (1 Person * 60 H * 21 Months). The Ordering Party Reserves That The Given Number Of Hours Is The Maximum Number, Which May Be Reduced Depending On The Actual Needs Of The Ordering Party, And The Contractor Will Not Be Entitled To Any Claims In This Respect.
