1. The Subject Of The Order Is The Provision Of 24-Hour Sanitary Transport Services For Repta Upper Silesian Rehabilitation Center. Gen. Jerzy Ziętek In Tarnowskie Góry.2. As Part Of The Subject Of The Order, The Contractor Is Obliged To Ensure Readiness To Provide Services 24 Hours A Day, 7 Days A Week, Also On Non-Working Days And Holidays - 24/7, In Accordance With The Received Service Order And Full Availability To The Ordering Party.3. Subject Of The Order Part No. 1 - Provision Of Sanitary Transport Services With The S Specialist Team 3.1.1. The Subject Of The Order Is The Provision Of Sanitary Transport Services With The S Specialist Team, Understood As The Transport Of Patients In A Life-Threatening Condition (Transport For Consultations And Diagnostic Tests, Transfer Of The Patient To Another Healthcare Entity).3.1.2. Approximate Required Quantities: Total: Approx. 2,500 Km/12 Months And Approx. 150 Hours/12 Months. The Contractor Is Obliged To Price The Offer Based On The Estimated Quantities, However, The Ordering Party Stipulates That The Contract Will Be Implemented In Accordance With Current Demand. The Contractor Is Not Entitled To Claim For The Provision Of The Service In The Amounts Specified In The Description Of The Subject Of The Order. The Ordering Party Reserves The Right To Make Quantitative Changes. The Ordering Party Reserves The Right To Limit The Order To 20% Of The Value Of The Subject Of The Agreement, Which Does Not Constitute Even Partial Withdrawal From The Agreement.3.1.3. The Maximum Waiting Time For The Execution Of A Transport Order Is 90 Minutes From Submitting The Order.3.1.4. Requirements For Ambulance Staff: A Specialist Team Consisting Of At Least Three People Authorized To Perform Emergency Medical Activities, Including A System Doctor And A System Nurse Or Paramedic As Defined In Art. 36 Section 1 Point 1 Of The Act Of September 8, 2006 On State Emergency Medical Services (Consolidated Text: Journal Of Laws Of 2026, Item 141) In The Event That None Of The Members Of The Emergency Medical Teams Meet The Conditions Referred To In Art. 106 Section 1 Of The Act Of January 5, 2011 On Vehicle Drivers (Consolidated Text: Journal Of Laws Of 2025, Item 1226, As Amended), The Fourth Person In The Team May Be The Driver. The Ordering Party Allows The Driver To Be Replaced By A Paramedic Authorized To Drive Emergency Vehicles.4. Requirements For Means Of Sanitary Transport: A) Technically Efficient Vehicles, I.E. They Have Current Technical Tests Admitting Them To Road Traffic; B) Meet Additional Conditions For Emergency Vehicles Specified In Detail In The Regulation Of The Minister Of Infrastructure Of December 31, 2022 On The Technical Conditions Of Vehicles And The Scope Of Their Necessary Equipment (Journal Of Laws Of 2024, Item 502, As Amended); C) Meet The Requirements Of Art. 36 Section 2 Of The Act Of September 8, 2006 On State Emergency Medical Services (Consolidated Text: Journal Of Laws Of 2026, Item 141)
