Tenders Are Invited For Implant Delivery

1. The Subject Of The Order Is The Delivery And Delivery To The Place Indicated By The Ordering Party Of Implants, In The Range And Quantities Specified In Annex 2 To Swz - Assortment And Price Form. 2. The Subject Of The Contract Is Divided Into 9 Parts/Tasks. 3. As Part Of The Task, The Contractor Is Obliged, Among Others, To: To: 1) Maintain A Sub-Warehouse/Deposit Of The Delivered Products, 2) Deliver The Subject Of The Order Within 48 Hours Of Submitting The Request, 3) Provide The Ordering Party With New Instruments, 4) Enable The Expansion Of The Original Instrumentation With Extensions And Augments, 5) Ensure A Full Deposit Of Implants, Consisting In Ensuring After Min. 1 Piece Of Size,7) Free Use Of Drives For The Procedure,8) Ensuring The Availability Of A Representative For The Surgical Procedure At The Operators Request,9) Conducting Training For Doctors And Nurses.4. The Subject Of The Order Should: A) Comply With Polish Standards Transposing European Standards Or Standards Of Other Member States Of The European Economic Area Transposing These Standards. B) Have Valid Documents Allowing For Marketing In The Territory Of The Republic Of Poland In Accordance With The Provisions Of The Applicable Act Of April 7, 2022 On Medical Devices (Journal Of Laws Of 2024, Item 1620), Its Transitional And Implementing Provisions And Eu Regulation 2017/745 On Medical Devices - Mdr (If The Law Imposes An Obligation To Have Such Documents) - If It Applies To A Given Product, C) Meet The Requirements Specified By The Ordering Party In The Specification Of Order Conditions, In Particular The Conditions Specified In The Description Of The Subject Of The Order. 5. The Task Should Be Completed Comprehensively And Completely From The Point Of View Of The Purpose It Is To Serve. 6. As Part Of The Execution Of The Order, The Contractor Is Obliged To Provide Additional Services (E.G. Transport, Insurance During Transport, Loading, Unloading, Etc.) Necessary For The Proper Execution Of The Order. 1. The Quantities Of Individual Products Given In The Assortment And Price Form - Annex No. 2 To The Swz Constitute The Estimated Quantity To Be Purchased During The Term Of The Contract. The Final Quantity Will Result From The Actual Needs Of The Ordering Party During The Order Execution Period. The Quantity Of The Ordered Goods May Change During The Term Of The Contract As Part Of Deliveries Interchangeably Accounting For The Total Gross Value Of The Contract. The Contractor Declares That He Will Not Pursue Any Claims From The Ordering Party In This Respect. 2. A Detailed Description Of The Subject Of The Order Is Provided In Appendix No. 2 To The Swz - Product And Price Form And The Draft Contract, Which Constitutes Appendix No. 5 To The Swz.
