The Subject Of The Order Is The Provision Of Medical Services To Employees And Phd Students Of The Institute Of Organic Chemistry Of The Polish Academy Of Sciences And Their Family Members. The Subject Of The Order Includes In Particular: A) Medical Services In The Field Of Occupational Medicine, Including The Provision Of Medical Services Including Laboratory And Diagnostic Tests To The Extent Necessary To Issue Medical Certificates And Specialist Consultations, Which Will Cover Employees And/Or Doctoral Students Of The Ordering Party, As Well As Persons Hired For Work; B) Medical Services In The Field Of Additional Medical Care, Including Comprehensive Medical Care, Including Comprehensive Outpatient Procedures, Including: Medical Consultations (In Stationary Form And In The Form Of Teleconsultation) Performed Without A Referral (Without Limit), Diagnostic Tests, Laboratory Tests, Comprehensive Diagnostic Tests, Pregnancy Management, Protective Vaccinations, Which Will Cover The Ordering Partys Employees And Their Closest Family Members, I.E. Authorized Persons, Within The Meaning Of Point Ii.3 Of The Opz (Appendix No. 1 To The Swz). A Detailed Description Of The Subject Of The Order Is Attached As Annex No. 1 To The Swz.
