1. The Subject Of This Order Is The Provision Of Services In The Field Of Testing For Sanitary And Epidemiological Purposes For Students Of The University Of Warmia And Mazury In Olsztyn.2. As Part Of The Service Constituting The Subject Of The Order, The Contractor Will Be Obliged To Perform Medical Examinations And Additional Tests (Chest X-Ray, Usr And Others According To The Doctors Recommendations) For Sanitary And Epidemiological Purposes Of The Students Of The Above-Mentioned Students. Faculties Of The University Of Warsaw In Olsztyn.3. In The Case Of Tests Described In Point 2, Based On The Medical Examination And The Results Of Laboratory Tests, The Doctor Issues A Medical Certificate For The Student Confirming The Lack Of Contraindications, Temporary Or Permanent Contraindication To Perform Work Or Study, Documenting The Result In The Examination Card For Sanitary And Epidemiological Purposes. The Results Of Laboratory Tests And The Content Of The Certificate Are Entered By The Doctor In The Test Booklet For Sanitary And Epidemiological Purposes And Forwarded To The Student.
