The Subject Of The Contract Is The Provision Of Care Services For 7 People, Residents Of The Komarówka Podlaska Commune, Including 5 People Who Need Support In Everyday Functioning, Who Are At Risk Of Being Placed In 24-Hour Institutions, People With Disabilities, Adults And Children, People At Risk Or Affected By Poverty Or Social Exclusion By 5 Guardians (1 Guardian Per 1 Person) And For 2 People Additionally Due To Their Special Social And Health Needs By 1 Guardian (1 Guardian Per Person). 2 People) - Participants Of The Project Titled Local Support System - Development Of Care Services In The Komarówka Podlaska Commune Implemented Under The European Funds For Lubelskie 2021-2027 Program Co-Financed By The European Social Fund Plus. The Project Is Implemented By The Municipal Social Welfare Center In Komarówka Podlaska.3.2. Care Services Include Assistance In Meeting Everyday Life Needs, Hygienic Care, Care Recommended By A Doctor And, If Possible, Ensuring Contacts With The Environment - Art. 50 Section 3 Of The Act Of 12 March 2004 On Social Assistance. When Providing Care Services, The Contractor Should Take Into Account The Subjectivity Of Service Recipients, Including Respecting The Right To Respect And Protection Of Dignity, Intimacy, In Particular In The Case Of Hygiene And Personal Care Activities, As Well As The Sense Of Security And Protection Of Personal Rights. The Scope Of Care Services Tailored To The Individual Needs Of The Project Participant Provided At The Place Of Residence Includes In Particular: 1) Meeting The Daily Life Needs Of The Patient, I.E.: A) Providing Meals For The Patient: Cooking Or Bringing Dinner, Feeding, B) Assistance In Keeping The Person Under Care Clean: The Patients Toilet, Including Hygiene Of The Oral Cavity, Fingernails And Toenails, Making The Bed, Laying The Patient Down And Preventing The Development Of Bedsores, Assistance With Physiological Activities, C) Maintaining Cleanliness In The Rooms Used By The Person Using The Aid And Sanitary Equipment And Sanitary Facilities, D) Washing Light Clothing And Personal Underwear, E) Shopping For Basic Groceries And Household Products In The Store, F) Collecting And Delivering Aid In Kind, Food Parcels, G) Walking Together And Contact With The Surroundings, 2) Visits With People Under Care In: Health Care Facilities, Institutions, Offices, Etc., A) Cooperation With A Health Clinic And A Community Nurse, Ordering Medical Visits, B) Collecting And Filling Prescriptions (For People Who Dont Leave Home), C) Handling Official Matters.3.3. Estimated Number Of Hours Of Care Services Provided: 5,760 Hours (40 Hours Per Month X 24 Months X 6 Caregivers). 1) Frequency And Duration Of Support: For 5 People, 4,800 Hours Each In 24 Months And For 2 People, 960 Hours In 24 Months. 2) Number Of People Under Care - 7 People. Organization Of The Activity: 7 People Will Receive An Assigned Caregiver Who Will Help Them Meet Their Daily Needs. By An Hour Of Service Provision
