The Subject Of The Order Is The Delivery, Assembly And Full Commissioning Of A Single-Plane Angiograph Along With The Modernization Of Rooms And Equipment For The Interventional Radiology Laboratory. A Detailed Description Of The Subject Of The Order (Technical Parameters Of The Angiograph) Is Attached As Annex No. 1 To The Swz. The Price Form Constitutes Annex No. 1A To The Swz. Guidelines Related To Modernization And Equipment Constitute Annex No. 1B To The Swz. Requirements Regarding The Subject Of The Order: 1. Delivery, Assembly, Installation And Full Commissioning Of The Single-Plane Angiograph With Complete System, Application And Database Software. 2. The Contractor Will Prepare A Concept For The Installation Of The Apparatus And The Functions Of The Adjacent Rooms And Submit It To The Ordering Party Within 7 Business Days Of Signing The Contract. The Contractor Will Provide The Ordering Party With The Necessary Documentation And Technical Conditions For The Workshop Regarding The Supplied Equipment, Which Are Necessary For The Proper Implementation Of Design Works And Adaptation Works Of The Workshop And Accompanying Rooms Within 7 Business Days From Signing The Contract. 3. Modernization Of The Interventional Radiology Laboratory With Equipment In Accordance With The Guidelines Contained In Annex No. 1B To The Swz. 4. Preparation Of A Technical Opinion, Necessary Design Documentation For The Replaced Installations And As-Built Documentation (Measurements, Tests, Certificates, Etc.). 5. Development And Implementation Of Appropriate Radiological Protection And Security Projects, Together With Appropriate Arrangements And Approval By The State Sanitary Inspectorate. 6. Conducting Acceptance Procedures In Accordance With Applicable Law In Order To Enable Use And Operation. 7. Performing Basic, Acceptance/Acceptance And Specialized Tests (Including Dosimetric Measurements Of The Effectiveness Of Radiological Shields) Of The Offered Angiograph, Monitors, And Equipment Supplied With The Device, Immediately After Its Installation. 8. The Contractor Will Secure, On Its Own And At Its Own Expense, All Materials Necessary To Complete The Subject Of The Order. The Offer Must Include All Costs Related To The Proper Implementation Of The Turnkey Order. 9. In Terms Of Safety Of Use, The Specified Medical Equipment Should Meet The Requirements In Accordance With The Act Of April 7, 2022 On Medical Devices (Journal Of Laws, Item 974). 10. As Part Of The Order, The Contractor Will Conduct Training, In Which Each Participant Will Receive A Certificate Confirming Their Qualifications To Operate The Device: A) Basic Training For 5 Doctors And 5 Technicians At The Ordering Partys Headquarters, Immediately After The Ordering Party Obtains Permits From The State Sanitary Inspectorate For A Period Of 4 Business Days; B) Advanced Training Of Medical Staff (5 People) At The Ordering Partys Headquarters For A Period Of Min. 4 Business Days Within The Period Agreed With The Ordering Party Within A Period Of Max. 6 Months From The Date Of Receipt Of The Entire Order. 11. The Contractor Is Obliged To Provide All Necessary Supplies
