1) Part No. 1 - Health Services In The Field Of Diagnostic And Preventive Consultations, Diagnostic Tests And Other Health Services Resulting From The Needs Of Project Participants. The Order Includes, Among Others: The Subject Of The Order Is The Provision Of Diagnostic, Preventive And Supportive Health Services Provided To The Project Participants, In Accordance With Their Individual Health Needs, Functional Status And The Notification Submitted By The Ordering Party. The Services Are To Be Provided Taking Into Account The Needs Of Older People, People With Disabilities, Lonely People, People Excluded From Communication And People In Need Of Support In Everyday Functioning. Services Will Be Provided To Two Groups Of Recipients: 1) To 22 Project Participants - In The Field Of Diagnostic And Preventive Health Services, Including In Particular: • Psychiatric Consultations, • Therapeutic Consultations, • Dietary Consultations, • Neurological Consultations, • Consultations Geriatrics, • Diagnostic Tests, • Medical Laboratory Diagnostics, • Other Health Services Resulting From The Individual Needs Of The Project Participant. Dimension: The Total Maximum Number Of Services For This Group Is 180 Hours, And Within This Pool Consultations And Diagnostic Tests May Be Carried Out, In Accordance With The Needs Of Project Participants.2) For 8 People, I.E.: • 4 Informal Caregivers, • 4 People Who Need Support In Everyday Functioning, In Particular: • Diagnostic And Preventive Consultations, Including Psychiatric Consultations, • Rehabilitation Services, • Other Health Services Resulting From The Participants Needs. The Type, Scope And Number Of Services Provided To Individual Project Participants Will Be Determined Each Time By The Ordering Party In Accordance With The Individual Health Needs Of The Participant. Dimension: 20 Visits/Treatments X 8 Upa One Visit/Treatment Is Understood As A One-Time Provision Of Health Services Intended For The Participant In Accordance With The Agreed Scope Of Support
