Tenders Are Invited For Providing Home Care Services In The City Of Racibórz For 20 People Dependent Participants Of The Project Entitled Good Autumn Of Life- Care For Residents 60+ From The City Of Racibórz - Efs Plus

1. The Subject Of The Order Is The Provision Of Home Care Services In The City Of Racibórz For 20 Participants In The Amount Of 9089 H.2. The Order Is Co-Financed By The European Social Fund Plus Under The Regional Program European Funds For Silesia 2021-2027, Priority Fesl.07 European Funds For Society, Measure 07.04 Social Services Under The Project Good Autumn Of Life - Care For Residents 60+ From The City Of Racibórz.3. Home Care Services Should Be Provided While Maintaining The Subjectivity Of Dependent Persons And Their Independence.4. An Order To Provide Care Services For A Specific Person Will Be Submitted To The Contractor In Writing, Electronically Or By Phone. The Contractor Will Receive From An Employee Of The Social Welfare Center In Racibórz Information Containing The Participants Data, I.E. Name, Surname, Address Of Residence Or Stay, Telephone Number (If Any), Scope Of Services Provided, Days Of Service Provision And Hours Of Service, Period Of Service Provision, I.E. Start Date And End Date Of Service Provision, Clients Payment Per Hour Of Services Provided - According To Which The Contractor Will Cover The Client With The Service Within The Periods And Hours Specified In The Administrative Decision.5. The Scope Of Activities Performed As Part Of The Home Care Service Will Be Individually Selected To The Needs, Potential And Personal Preferences Of Project Participants And Will Include: A) Assistance In Meeting Everyday Life Needs; B) Hygiene Care; C) Care Recommended By A Doctor, Which Includes Care Activities Resulting From The Submitted Medical Certificate Or Medical Documentation, Supplementing Community Nursing Care; D) Ensuring Contacts With The Environment.6. During The Hours Of Home Care Services, Other Forms Of Service Assistance Cannot Be Provided, Including: Assistant Services Or Specialist Care Services Referred To In The Act Of March 12, 2004 On Social Assistance (Journal Of Laws Of 2025, Item 1214).7. The Scope Of Services Will Result From The Needs Of People Using Assistance And Will Be Determined Individually, Provided That: A) Services Will Be Provided On Average. 64 H/Month Per Participant (Average 4 Visits A Week, 4 Hours Each). B) Care Services Will Ensure Availability Of Uninterrupted And Appropriate Quality Of Service Provision Process 7 Days A Week, By Agreeing With Caregivers The Hours And The Ordered Size And Scope Of Services. C) The Organization Of The Provision Of Home Care Services Takes Into Account The Subjectivity Of Service Recipients, Including Respecting The Right To Respect And Protection Of Dignity, Intimacy, In Particular In The Case Of Activities Of A Hygienic And Personal Care Nature, As Well As The Sense Of Security And Protection Of Personal Rights. D) One Hour Means One Clock Hour. The Ordering Party Emphasizes That The Term Hours Of Service Provision Should Be Understood Only As The Actual Time Of Service Provision. 8. The Contractor Will Be Fully Liable For Any Damage Caused By Persons
