Tenders Are Invited For Providing Sanitary Transport Services For Patients Of The 6Th Military Hospital With The Sp Zoz Outpatient Clinic In Dęblin Using An A2 Road Ambulance With A Basic Team

1. The Subject Of The Order Is The Provision Of Sanitary Transport Services For Patients Of The 6Th Military Hospital With A Spzoz Outpatient Clinic In Dęblin By An A2 Road Ambulance With A Basic Team To Other Units Providing Medical Services For The Purpose Of Further Treatment Or Examination Or Consultation Or Transport Of Patients Home, In Accordance With The Needs Of The Ordering Party, In The Amount Of 20,000 Km And 500 Hours. This Service Must Be Available 24 Hours A Day, 7 Days A Week, At The Request Of The Ordering Party. 2. The Ordering Party Reserves The Right Not To Use The Full Number Of Hours And Kilometers Contracted. In Such A Case, The Contractor Will Not Be Entitled To Claim Compensation For The Damage Caused By This Reduction. Such A Reduction Cannot Constitute A Basis For The Contractor To Request A Change In The Terms Of The Contract, Including The Price. 3. The Means Of Sanitary Transport Used Must Meet The Conditions Specified In The Regulation Of The Minister Of Infrastructure Of December 31, 2002 On The Technical Conditions Of Vehicles And The Scope Of Their Necessary Equipment (Consolidated Text, Journal Of Laws Of 2016, Item 2022, As Amended) And The Requirements Described In The Act Of April 15, 2011 On Medical Activities (Consolidated Text: Journal Of Laws Laws Of 2026, Item 156) Properly Marked, Equipped With Appropriate Equipment In Accordance With Applicable Regulations (Polish Standards Transposing European Harmonized Standards, In Accordance With The Provisions On State Emergency Medical Services). Means Of Transport Must Have A Valid Third Party Liability And Accident Insurance Package. The Vehicles Production Year Should Not Be Earlier Than 2014. 4. Persons Performing The Service Must Have Appropriate Authorizations And Professional Qualifications. 5. As Part Of The Sanitary Transport Service By Means Of An A2 Road Ambulance With A Basic Unit Within The Meaning Of Art. 36 Section 1 Point 2) Letter A) The Act Of September 8, 2006 On State Emergency Medical Services (Consolidated Text: Journal Of Laws Of 2025, Item 91) The Contractor Is Obliged To: - Pick Up The Patient From The Ward/Clinic And Bring Or Possibly Take Him To The Place Indicated By The Ordering Party; – Transport To And From Examinations And Clinics (Ct And Mri Laboratories, Diagnostic Consultations, Transport Of The Patient To The Indicated Hospital, Transport To The Place Of Residence, Etc.); – Assisting Patients During Transport; – In Special Cases, The Transport Service May Include The Transport Of Biological Materials And Other Materials Used To Provide Health Services And The Transfer Of Medical Documentation. 6. Sanitary Transport Carried Out At The Request Of The Ordering Party Will Be Carried Out Using Wireless Communication Means, Enabling Direct, Continuous Contact With The Driver.
