1. Order Completion Date: From April 1, 2026 To December 31, 2027, Subject To The Provisions Of Point 17 And Point 19.2. The Contractor Undertakes To Prepare Meals In Accordance With The Principles Set Out In The Act Of August 25, 2006 On Food And Nutrition Safety, Including The Implementing Provisions To The Above-Mentioned. Act. 3. Meals Must Be Prepared From Wholesome Products, Including Fresh Seasonal Vegetables, Before Their Expiry Date.4. When Delivered To The Authorized Person, Meals Must Be Hot And Ready For Immediate Consumption, Without The Need To Reheat Them.5. The Minimum Caloric Value Of The Meal Must Be 850 Kcal.6. Meals Must Be Varied, And Individual Dishes May Be Repeated After 10 Calendar Days.7. The Meal Must Consist Of A Hot Dinner Dish*: A) Meat - At Least 3 Times A Week, B) Semi-Meat, E.G. Dumplings With Meat, Dumplings With Cabbage And Meat, Dumplings With Meat - No More Than Once A Week, C) Meat-Free E.G. Eggs, Potato Dumplings, Potato Pancakes - No More Than Once A Week, D) Hot Soup In An Amount Of At Least 500 Ml + 150 G Insert (Sausage, Meat) And 4 Slices Of Bread - Served Twice A Week. * Or According To The Assessed Criterion Proposed In The Offer8. Meals Will Be Delivered On The Basis Of A List Of Names Of Persons Entitled To Meals Provided By The Ordering Party, Along With Individual Attendance Cards, Submitted 2 Business Days Before The Beginning Of The Month In Which The Meals Will Be Provided. The List Of Cards Will Be Sent Via E-Delivery, And In The Absence Of Them, In The Form Of An Encoded Email Sent To The Contractor. 9. Meals Will Be Delivered By Transport Provided By The Contractor, Free Of Charge, To The Place Of Residence Of Eligible Persons In Gliwice. Each Time The Meal Is Served, It Must Be Confirmed By The Signature Of An Authorized Person. If The Authorized Person Is Unable To Sign, The Delivery Of The Meal Is Confirmed By The Person Providing Direct Care At The Time Of Delivery Of The Meal Or By The Meal Supplier. In Such A Situation, The Contractor Places The Note Signature Impossible On The Card Along With The Signature Of The Person Receiving The Meal And An Indication Whether It Is An Authorized Person Or The Contractors Employee Delivering The Meal.10. Each Time After Completing The Delivery Of Meals, In The Absence Of An Authorized Person, The Contractor Shall Write The Word Absent In Place Of The Signature.
