The Subject Of The Order Is The Provision Of 920 Hours Of Psychologist Services Conducting Consultations For Dependent Persons, Seniors And Informal Caregivers In A Designated Place In The Stary Dzierzgoń Commune, Pomeranian Voivodeship, Including: A) Psychologist Service Provided In The Place Of Residence Of Dependent Persons, I.E. Seniors, People With Disabilities, Single People, Chronically Ill: Number Of People Supported: Minimum 20; Number Of Hours Per Project Participant: On Average 22 Hours (Average 1.5 H/Month) Number Of Service Hours Covered By The Contract: 440 H (Hour = 60 Minutes). The Specialized Service Will Be Provided With Access To The Individual Places Of Residence Of Dependent Persons, I.E. In The Area Stary Dzierzgoń Commune, From Monday To Friday From 8.00 A.M. To 5.00 P.M. The Schedule Of Individual Journeys To Destinations Will Vary, Depending On The Recruited Participants. B) Psychologist Service As Part Of Group And Individual Support, Provided At The Day Care Center In Stary Dzierzgoń For Seniors And People With Disabilities: - Number Of People Supported: Minimum 15; - Number Of Hours Of Group Support: On Average 1 Meeting Per Month - 2.5 Hours; - Number Of Hours Of Individual Support: On Average. 12.5 Hours Per Month;- Number Of Hours Of Service Covered By The Contract: 240 H (One Hour = 60 Minutes). For Classes, The Ordering Party Will Provide The Contractor With A Room Free Of Charge. C) Psychologist Service Provided To Informal Caregivers Of People Who Need Support In Everyday Functioning:- Number Of People Covered By Support: Minimum 20;- Number Of Hours Of Support: Average 15 Hours Per Month;- Number Of Hours Of Service Covered By The Contract: 240 H (Hour = 60 Minutes).The Service For Informal Caregivers Of People In Need Of Support Will Be Provided In The Premises Of The Municipal Social Welfare Center In Stary Dzierzgoń, With The Exception Of A Maximum Of 60 Hours Of Service, Which Will Be Provided At The Place Of Residence Of The Person Receiving Support. The Room For Individual Classes Will Be Made Available Free Of Charge To The Service Provider.2.
