Supplier should furnish a written request and a receipt issued by the Accountant of Sri Jayewardenepura General Hospital for the payment of Registration fee of Rs. 2,500.00 by cash for each category to collect the application. Applications should be sent under registered cover addressed to the Chairman, Sri Jayewardenepura General Hospital, Thalapathpitiya, Nugegoda, marked "Registration of Suppliers for year 2026/2027" on the left hand top corner of the envelope. Application for each category should be sent under one cover indicating the category of supply applied for on the envelope on or before 06.04.2026.
