1. The Subject Of The Order Is The Lease And Operation, Subject To Option Rights, Of 680 Pieces Of Oxygen Concentrators (For Home Use) (Hereinafter Referred To As Concentrators, Medical Equipment Or The Subject Of The Lease), For The Needs Of Patients Of The Cpit In Bystra Receiving Oxygen Therapy At Home As Part Of Services Financed By The National Health Fund, Meeting The Requirements Specified In The Provisions Of The Swz And The Annex. No. 2 To Swz – List Of Technical Parameters. The Subject Of The Order Will Be Implemented On The Terms And Conditions Specified In The Draft Contract Constituting Annex. No. 5 To Swz2. Oxygen Concentrators Must Be Fully Operational, Fit For Use, Approved For Trading And Use, And Should Meet All The Requirements Specified In Applicable Law, Including The Act Of April 7, 2022. On Medical Devices (Consolidated Text: Journal Of Laws No. 2024, Item 1620), Have Eu Declarations Of Conformity And Those Marked With The Ce Mark Manufactured No Earlier Than In 2020. 3. At The Time Of Initiation Of The Proceedings In Question, The Home Oxygen Treatment Program (Hereinafter Referred To As Dlt) Covers Patients From The Silesian Voivodeship (Counties: Żywiec, Pszczyna, Bieruńsko-Lędziński, Bielsk, Cieszyn, Zawiercie, Wilamowicki, Ślemień, And From The Cities Of: Dąbrowa Górnicza, Tychy, Katowice, Świętochłowice, Rybnik, Małopolska (Counties: Oświęcimski, Wadowicki, Chrzanów, Suski). 4. The Place Of Execution Of The Order Will Be The Place Of Residence Of The Patients Indicated By The Ordering Party - Users Of The Oxygen Concentrators.
