1. The Subject Of The Order Is A Service Consisting In: 1) Castration Of Free-Living Cats From The Góra Kalwaria Commune (Estimated Number Of 100 Pieces), 2) Provision Of A Service Consisting In Euthanizing Blind Litters Of Free-Living Cats From The Area Of Góra Kalwaria Commune (Estimated Number Of 12 Pieces), 3) Treatment Of Free-Living Cats From The Góra Kalwaria Commune, Including: A) Clinical Ultrasound Examination (Estimated Number Of 3 Pcs.)B) X-Ray Clinical Examination (Estimated Quantity 10 Pieces)C) Basic Blood Test (Estimated Quantity 20 Pieces)D) Extended Blood Test (Estimated Quantity 20 Pieces)E) Tests For Infectious Diseases (Estimated Quantity 20 Pieces)F) Wound Dressing (Estimated Quantity 10 Pieces)G) Surgical Procedures (Estimated Quantity 2 Pieces)H) Day Of Hospitalization. (Estimated Quantity 10 Pieces).2. Performing The Treatments Specified In Section 1 Should Be Performed In Accordance With Applicable Law, Good Veterinary Practices, Own Knowledge And Experience, In A Way That Minimizes Animal Suffering.3. Performing The Treatments Referred To In Section 1 Will Take Place In A Veterinary Facility That Meets The Requirements Of Applicable Law.4. Organizing The Working Time Of The Veterinary Facility In A Way That Ensures The Implementation Of The Subject Of The Order, In Accordance With The Submitted Offer. 5. The Contractor Will Provide Appropriate Pharmacological Agents, Equipment And Conditions For The Proper Performance Of The Subject Of The Order.6. Placing In A Visible Place On The Premises Of The Veterinary Facility Information Regarding The Veterinary Service Covered By This Agreement Performed At The Expense Of The Góra Kalwaria Commune.7. Performing The Treatments Specified In Section 1 Will Be Performed Only By A Veterinarian Who Has The Active Right To Practice The Profession In Accordance With The Act Of December 21, 1990 On The Profession Of Veterinarian And Veterinary Chambers (Consolidated Text: Journal Of Laws Of 2026, Item 125, As Amended) Or Who Has The Ability To Provide Services In Accordance With Art. 2H Of The Given Act.8. Before Starting The Castration Procedure, The Contractor Is Obliged To: 1) Perform A Preliminary Examination To Determine Whether The Animals Health Condition Allows The Procedure To Be Performed; 2) Set A Date For The Procedure Directly With The Guardian Registered As The Guardian Of Free-Living Cats In The Góra Kalwaria Commune; 3) Confirmation Of The Provision Of The Veterinary Service On The Referral Issued By The Ordering Partys Representative For The Provision Of The Veterinary Service By: A) Placing An Annotation On The Referral About The Provision Of The Service With The Date, Signature And Stamp Of The Veterinarian; B) Confirmation Of The Provision Of The Veterinary Service By The Registered Guardian Of Free-Living Cats With The Date And Signature. 9. The Contractor May Not Charge Animal Owners Any Additional Fees Related To The Treatments Performed Under This Contract.10. Th
