The Subject Of The Order Is The Implementation Of Tasks In The Field Of Government Administration Resulting From The Program Of Comprehensive Support For Families For Life In The Scope Of Ensuring The Implementation Of The Tasks Of The Leading Coordination, Rehabilitation And Care Center In 2026 In The Świecie County, Consisting In Multi-Aspect And Comprehensive Assistance To A Disabled Child In The Period From 0 Years Of Age To Starting School And His/Her Family.2. The Tasks Of The Leading Coordination, Rehabilitation And Care Center Under The Agreement Concluded By The Świecki County Are Fully Financed From The State Budget As Part Of The Implementation Of Tasks In The Field Of Government Administration Resulting From The Comprehensive Support Program For Families For Life Constituting An Annex To Resolution No. 189 Of The Council Of Ministers Of December 27, 2021 On The Comprehensive Support Program For Families For Life (M.P. Of 2022 R. Item 64).3. The Institution Directly Implementing The For Life Program Is The Special Kindergarten No. 10 Słoneczna Polana For Children With Intellectual Disabilities, Disabilities And Autism In Świecie In The Special School Complex No. 1 In Świecie, Located At Ul. Paderewskiego 5A, 86-105 Świecie. 4. The Person Responsible For The Proper Implementation Of The For Life Program, In Particular For Preparing Class Schedules And Allocating Hours For All Specialists, As Well As Settling Classes, Is The Program Coordinator - Vice-Principal Of The Special School Complex No. 1 In Świecie.5. Classes Under The For Life Program Will Be Carried Out: 1) At The Special School Complex No. 1, Ul. Paderewskiego 5A, 86–105 Świecie;2) In Specialized Offices Of Educational Institutions And Units In The Świecie District;3) Conditionally In Rooms Available To The Contractor, If Necessary, And The Contractor Will Demonstrate The Appropriate Equipment Necessary To Carry Out Therapeutic Tasks And Obtain The Consent Of The Program Coordinator.6. The Number Of Hours To Be Completed By One Specialist Will Depend On The Needs And Disabilities Of The Child. However, The Program Provides For Working With One Child Up To 5 Hours A Week, The Number Of Hours Depending On The Needs Of A Given Child. Support Will Be Provided In Accordance With The Submitted Application As Part Of The Recruitment Process. 7. The Number Of Hours To Be Worked Will Depend On The Number Of Registered Children And The Type Of Diagnosed Disability, Provided That The Above-Mentioned Number Of Hours For Specific Specialists Are The Maximum.
