The Subject Of This Procedure Is The Successive Delivery Of Specialized Cleaning Agents And Preparations For Surface Disinfection, Skin Care And Disinfection, Divided Into The Following Tasks: Task No. 1: Surface Disinfection Preparations, Skin Care And Disinfection2. The Detailed Scope And Description Of The Subject Of The Order, Including The Type And Quantity Of Products, Are Specified In The Assortment And Price Form Constituting Annex No. 1 To The Swz. This Description Contains The Minimum Requirements Of The Ordering Party, Which Means That The Offered Products Must Meet All The Parameters Specified In This Form. The Description Of The Subject Of The Order Should Be Read Together With Any Changes To The Content Of The Swz, In Particular Resulting From The Answers To The Contractors Questions.3. The Quantities Provided In The Assortment And Price Form Constitute The Estimated Demand During The Term Of The Contract And Are Used Only To Calculate The Offer Price, I.E. To Determine The Maximum Remuneration For The Contractor. The Purchase Of Individual Products Will Depend On The Actual Needs Of The Ordering Party. The Ordering Party Reserves The Right To: • Reduce The Scope Of The Subject Of The Order, While Guaranteeing The Implementation Of The Contract At A Level Not Lower Than 50% Of The Order Value, • Changing The Quantity Of The Ordered Assortment Between Individual Items.4. The Offered Products Must Be: A) Brand New, Unused And Have A Minimum Expiry Date Of 12 Months From The Date Of Delivery, B) Delivered In Original, Intact Manufacturers Packaging, Protected Against Damage During Transport, C) Contain Clear Manufacturers Markings, Product Name, Batch Number (If Applicable), Date Of Production Or Expiry Date And Method Of Use, D) Provided With A Commercial Label In Polish, Containing The Necessary Information Needed For Safe Use For The Direct User, E) Approved For Trading And Use In The Territory Of The Republic Of Poland In Accordance With Applicable Legal Provisions In This Regard, Must Have: Pzh Certificates, Technical Approvals, Certificates Of Conformity, Product Data Sheets, Safety Data Sheets, Etc. Depending On The Type Of Product. The Ordering Party May Request Up-To-Date Documents Referred To Above At Any Time During The Term Of The Contract. 5. The Offered Products Must Be Intended For Use In Entities Performing Medical Activities. The Preparations Should Demonstrate Biocidal Effectiveness In Accordance With Applicable Standards (Including Against Bacteria, Fungi, Viruses, Mycobacteria - Depending On The Intended Use Of The Product) And Have The Permits And Entries In The Appropriate Registers Required By Law.6. If The Product Offered Is A Medical Device, It Must Meet The Requirements Specified In The Legal Provisions Regarding Medical Devices, In Particular Regulation (Eu) 2017/745 Of The European Parliament And Of The Council (Mdr) And The Medical Devices Act.
