Special-Purpose Road Passenger-Transport Services – Pn 06/26 Providing Patient Transport Services By Sanitary Transport With A T-Type Unit.. 3.1. The Subject Of The Order Is The Provision Of Services In The Field Of Transport Of Patients By Sanitary Transport With A T-Type Unit, In Accordance With The Description Of The Subject Of The Order And With The Relevant Contractual Provisions And The Instruction - Io 4-53-13 In Force At The Ordering Party, Constituting Annex No. 12 To This Swz. A Detailed Description Of The Subject Of The Order Is Provided In Annex No. 4 To Swz (Opz) - Description Of The Subject Of The Order. 3.2. Detailed Conditions And Rules For The Implementation Of The Subject Of The Order Are Additionally Specified In The Contractual Conditions Constituting Annex No. 5 To The Swz Contract Template. Before Signing The Contract, Contractors Jointly Applying For The Contract (If Their Offer Is Selected As The Most Advantageous) Will Present To The Ordering Party An Agreement Regulating The Cooperation Of These Contractors. Within 2 Days Before Signing The Contract, The Contractor Will Provide The Ordering Party With: Copies Of Motor Insurance Policies And Valid Registration Certificates Of The Above-Mentioned Vehicles, A Copy Of The Third Party Liability Insurance Policy For The Business Related To The Subject Of The Order For An Amount Not Less Than Pln 100,000.00, A List Of Persons Who Will Provide The Ordering Partys Service Along With A Photocopy Of Their Authorizations Certified As True Copies. 3.3. The Ordering Party Requests That The Contractor Indicate In The Offer The Parts Of The Order Whose Performance He Intends To Entrust To Subcontractors And That The Contractor Provide The Companies Of The Subcontractors - In Accordance With Art. 462 Section 2 Of The Public Procurement Law, If They Are Already Known. 3.4. If The Change Or Resignation From The Subcontractor Concerns The Entity Whose Resources The Contractor Relied On, In Accordance With The Principles Set Out In Art. 118 Section 1 Of The Public Procurement Act, In Order To Demonstrate Compliance With The Conditions For Participation In The Procedure, The Contractor Is Obliged To Demonstrate To The Ordering Party That The Proposed Other Subcontractor Or Contractor On Its Own Meets Them To An Extent No Less Than The Subcontractor Whose Resources The Contractor Relied On During The Procurement Procedure. The Provision Of Art. 122 Of The Public Procurement Law Shall Apply Accordingly. 3.5. Employment Requirements Based On The Employment Relationship: 3.5.1. Pursuant To The Content Of Art. 95 Of The Public Procurement Act, The Ordering Party Requires The Contractor Or Subcontractor Or Further Subcontractor To Employ, Under An Employment Relationship, Persons Performing Work That Includes The Following Types Of Activities Covered By The Subject Of The Order: I.E. Drivers, Employees (Ambulance Staff), If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974 - Labor Code (T.J. Journal Of Laws Of 2025, Item 277), Unless They Will Be Directly Performed By A Given Person As Part Of His Sole Proprietorship. Method Of Documentation, Verification, Employment Control Rights And Sanctions Resulting From Non-Compliance With The Above-Mentioned.
