Medical Information Systems – A System For Digitizing Documentation Of Anesthesia, Intensive Care Unit With An Operational Schedule Module And The Necessary Equipment. 1. The Subject Of The Order Is The Delivery And Implementation Of An Electronic Anesthesia Card System With Hardware Infrastructure And The Provision Of Warranty And Maintenance Services By The Contractor To The Ordering Party In Accordance With The Scope Specified In The Description Of The Subject Of The Order (Hereinafter Referred To As Opz) Constituting Annex No. 1 To The Swz. 2. Acting On The Basis Of Art. 257 Of The Act, The Ordering Party Informs That It May Invalidate This Contract Award Procedure If The Public Funds It Intended To Use To Finance The Entire Contract Or Part Of The Order Were Not Awarded To It. 3. The Ordering Party Does Not Envisage The Possibility Of Awarding Contracts Referred To In Art. 214 Section 1 Point 8 Of The Public Procurement Law. 4. The Ordering Party Requires A Security For The Proper Performance Of The Contract In The Amount Of 5% Of The Gross Offer Price. 5. Subjective Means Of Evidence. 5.1 The Ordering Party Requires The Submission Of The Said Piece Of Evidence Together With The Offer - The Contractors Declaration, In Accordance With The Template Constituting Annex No. 1A To The Swz, Confirming That The Offered Subject Of The Order Meets The Required Parameters. 5.2 The Evidence Referred To In Point 5.1 Should Be Submitted In Electronic Form, Bearing A Qualified Electronic Signature Of A Person Authorized To Represent The Contractor Indicated In The Appropriate Register Or Records, Or Of A Representative. 5.3 The Ordering Party Will Accept Equivalent Means Of Evidence If They Confirm That The Deliveries Offered Meet The Requirements Or Features Specified By The Ordering Party In Annex No. 1A To The Swz. 5.4 The Ordering Party Will Call On The Contractor To Submit Or Supplement The Evidence Within The Specified Deadline If The Contractor Has Not Submitted The Evidence Along With The Offer Or The Evidence Submitted Is Incomplete. 5.5 The Ordering Party May Request Explanations From The Contractor Regarding The Content Of The Evidence In Question. 5.6 The Ordering Party Will Not Request The Contractor To Submit Or Supplement The Evidence If, Despite The Submission Of The Evidence, The Offer Is Rejected Or There Are Grounds For Invalidating The Proceedings.
