The Subject Of The Order Is The Provision Of Medical Services In The Field Of Additional Medical Care For: A) Employees Of The Ordering Party - The Provincial Fund For Environmental Protection And Water Management In Warsaw, Hereinafter Referred To As The Employees Employees, As Part Of The Package:• Employee Package;B) Closest Family Members Of The Ordering Partys Employees, Hereinafter Referred To As Authorized Persons, As Part Of The Packages:• Partnership Package - Partner (Up To 67 Years Of Age) Or One Child, And• Package Family - Partner (Up To 67 Years Old) And/Or Children.2. Person Authorized To Use Medical Services In The Field Of Additional Medical Care For The Immediate Family Members Of The Ordering Partys Employees, Within The Meaning Of Section 1 Letter B) Is Respectively: A) The Employees Spouse/Partner, And B) The Employees Child/Children (Without Quantitative Limits). Note: The Employees Child/Children Are Understood As: The Employees Own Child, The Employees Spouse, From The First Day Of His Or Her Life, Who Is Under 18 Years Of Age On The Date Of Joining The Agreement, And In The Case Of The Child Attending School Within The Meaning Of The Law On Higher Education And Science (Journal Of Laws Of 2024 Item 1571, As Amended) Is Under 26 Years Of Age.3. By Providing Medical Services In The Field Of Additional Medical Care, The Ordering Party Understands Providing And Maintaining Readiness To Provide Medical Services Under The Conditions And Within The Scope Specified In Sopz.4. The Detailed Scope Of The Services Is Specified In Annex No. 1 To The Agreement, Constituting Annex No. 8 To The Swz.
