Medical Equipments – Purchase Of Medical Equipment, Devices And Medical Products. 1. The Subject Of The Order Is The Purchase Of Medical Equipment, Devices And Medical Products: 1) Wheelchair - 2 Pcs. 2) Exercise Test Set With Treadmill - 1 Set 3) Defibrillator With Accessories - 2 Sets 4) Spirometer - 1 Pc. 5) Mobile Ecg - 1 Set 6) Pressure Holter - 2 Pcs. 7) Patient Monitoring System For 8 Stations With Invasive And Non-Invasive Cardiac Output - 1 Set 8) Ultrasound Machine With Echocardiography Function - 1 Set 9) Portable Echocardiogram - 1 Set 10) Ecg Holter With Software - 6 Pcs. 11) Vital Signs Monitor Integrated With The Nxt Set - 2 Sets. The Subject Of The Order Is The Delivery, Installation, Commissioning And Training Of The Ordering Partys Staff, Medical Equipment And Supplies. 2. The Ordering Party Requires At Least A 24-Month Full Warranty For The Delivered Subject Matter. 3. The Equipment Must Be Registered Or Notified In Poland As A Medical Device And Marked With The Ce Mark. 4. The Contractor Hereby Guarantees That The Equipment Is Brand New (Year Of Production Not Earlier Than 2025), Unused, Complete And That It Is Not Necessary To Purchase Additional Elements And Accessories To Start It And Use It As Intended. No Apparatus Or Any Of Its Components, Accessories, Etc. It Is Not Reconditioned, Post-Exhibition Equipment And Has Not Been Previously Used By Another User. 5. Under This Order, The Contractor Is Obliged To Ensure Compliance Of The Delivery Of Medical Equipment With The Principle Of Not Causing Serious Damage To The Environment (Dnsh), In Accordance With Art. 17 Of Regulation (Eu) 2020/852 Of The European Parliament And Of The Council And The Document Guidelines For Beneficiaries On The Application Of The Dnsh Principle In Projects Financed By Eu Funds Set Out In Annex 3 To The Agreement. Conditions Of Order Execution - Dnsh Principle: The Contractor Is Obliged To Perform The Order In Compliance With The Principles Of Environmental Protection And Sustainable Development, In Accordance With The Dnsh Principle. In Particular, The Contractor Will Ensure That: A) Transport, Packaging And Delivery Methods Limit The Emission Of Pollutants And Waste, B) The Delivered Equipment Meets Environmental Requirements (Including Rohs, Reach, Energy Efficiency), C) The Waste Generated Will Be Managed In Accordance With The Regulations On Waste And Weee. The Ordering Party Reserves The Right - At The Stage Of Contract Implementation - To Check Compliance Of The Implementation With Dnsh Requirements. Principle Of Equality The Ordering Party Informs That The Implementation Of This Order Must Respect The Principle Of Equal Opportunities And Non-Discrimination, In Particular In Relation To: 1) Equal Treatment The Contractor Undertakes To: - Comply With The Principle Of Equal Treatment Of Persons, Regardless Of Their Gender, Age, Disability, Race, Ethnic Origin, Religion, Belief, Sexual Orientation Or Gender Identity, - Ensure That All Persons Involved In The Implementation Of The Order (Including Delivery, Training, Installation Staff, Service) Will Be T
